摘要
胃癌作为严重威胁我国居民健康的消化道恶性肿瘤之一,发病率与病死率近年来呈现持续上升趋势。现代医学主要采用手术切除、化学药物治疗及分子靶向疗法等治疗手段,虽取得一定临床疗效,但常伴随不同程度的药物不良反应以及治疗过程中的相关并发症。源自东汉张仲景《伤寒杂病论》的经典方剂半夏泻心汤由半夏、黄芩、黄连等7味药组成,以其寒热平调、辛开苦降的功效特点,恰与中医理论中胃癌“寒热错杂、气机失和”的核心病机相契合。研究表明,该方剂在调控肿瘤微环境、抑制癌前病变进展、阻断转移途径以及缓解术后或放化疗所致消化系统并发症等方面展现显著治疗优势。该文从半夏泻心汤抑制胃癌细胞的增殖和诱导其凋亡、阻止胃癌细胞进行腹膜内转移、修复胃黏膜屏障阻抑癌前病变、发挥抗氧化作用清除自由基、协同放化疗作用等方面进行论述,以期为半夏泻心汤对胃癌防治作用的深入研究提供理论支撑,并推动其在胃癌综合治疗中的规范化应用,为其在临床上的应用提供科学依据。
Gastric cancer,as one of the malignant tumors of the digestive tract that seriously threatens the health of Chinese residents,has shown a continuous upward trend in both incidence and mortality in recent years.Modern medicine primarily employs surgical resection,chemotherapy,and molecular targeted therapies,which have achieved certain clinical efficacy but are often accompanied by varying degrees of adverse drug reactions and treatment-related complications.Banxia Xiexin Decoction(半夏泻心汤),a classical formula originating from ZHANG Zhongjing's Treatise on Cold Damage and Miscellaneous Diseases in the Eastern Han Dynasty,is composed of seven herbs including Banxia(Pinellia Ternata),Huangqin(Scutellaria Baicalensis),and Huanglian(Coptis Chinensis).Characterized by its dual actions of harmonizing cold-heat imbalances and regulating Qi dynamics through pungent-dispersing and bitterdescending properties,this formula aligns with the traditional Chinese medicine(TCM)pathogenesis theory of gastric cancer as a“cold-heat complex with disrupted Qi harmony.”Research indicates that this decoction demonstrates significant therapeutic advantages in modulating the tumor microenvironment,inhibiting the progression of precancerous lesions,blocking metastasis pathways,and alleviating postoperative or chemotherapy/radiotherapy-induced gastrointestinal complications.This article elaborates on Banxia Xiexin Decoction's mechanisms in suppressing gastric cancer cell proliferation,inducing apoptosis,preventing intraperitoneal metastasis,repairing the gastric mucosal barrier to hinder precancerous lesions,exerting antioxidant effects to scavenge free radicals,and synergizing with chemo/radiotherapy.The aim is to provide theoretical support for in-depth research on its preventive and therapeutic roles in gastric cancer,promote its standardized application in comprehensive gastric cancer treatment,and offer scientific evidence for its clinical utilization.
