摘要
克罗恩病(CD)是一种慢性非特异性肠道炎症性疾病,其发病机制复杂,涉及遗传、免疫、环境等多因素相互作用。中医学认为,CD可归属于“肠痈”范畴,其中“脾为之卫”功能失常在CD发生发展中具有关键作用。《黄帝内经》提出“脾为之卫”理论,指出脾主运化水谷精微以充养卫气,卫气充盛则外邪难侵。现代研究表明,“脾为之卫”功能与肠道菌群的稳态及免疫调节等密切相关。基于此,本文从“脾为之卫”理论出发,探讨健脾类中药在平衡肠道菌群、免疫调节等方面针对CD的作用机制,为中医药防治CD提供新的理论依据和治疗策略。
Crohn's disease(CD)is a chronic non-specific intestinal inflammatory disease.The pathogenesis of CD is complex,involving the interaction of genetics,immune,environmental and other factors.Traditional Chinese medicine considers that CD can be classified into the category of"intestinal abscess",in which the dysfunction of"the spleen serves as the body's guard"plays a key role in the occurrence and development of CD.The"Huangdi Neijing"puts forward the theory of"the spleen serves as the body's guard",and points out that the spleen mainly transports and transforms the essence of food and water to nourish the defensive qi.When the defensive qi is abundant,external pathogens are hard to invade.Modern studies have shown that the function of"the spleen serves as the body's guard"is closely related to the homeostasis and immune regulation of intestinal flora.Based on this,this paper explores the mechanisms by which spleen-fortifying Chinese herbal medicines act on CD in terms of balancing intestinal flora and immune regulation,so as to provide new theoretical basis and treatment strategy for the prevention and treatment of CD by traditional Chinese medicine.
