摘要
中医学认为,阳虚为诸病产生的主要根源。基于扶阳理论及人体阴阳本体结构论,本研究认为慢性心力衰竭发生主要归根于机体“元阳亏虚,邪扰正衰”,元阳不足致使“阳化气,阴成形”的生理平衡失调,阳气温煦推动功能失司,阴寒痰瘀等病理产物过度积聚。由此,本病的基本病机可概括为“阳虚为本,阴实为标”。治以温扶阳气以消阴,遵循三焦次第治疗原则,初期采用桂枝法宣通中上焦阳气,恢复气机升降;中期运用四逆法温补固摄下焦元阳,培补先天之本;后期通过补益脾胃,以固后天化源。如此方可使阳气功用复常,密固于周身,最终实现“阴平阳秘,精神乃治”的治疗目标。
Traditional Chinese medicine believes that Yang deficiency is the main root cause of various diseases.Based on the theory of tonifying yang and the ontology of yin and yang in the human body,this study believes that the occurrence of chronic heart failure is mainly attributed to the body's"deficiency of yuan yang and disturbance of positive and negative energy".Insufficient yuan yang leads to physiological balance imbalance of"yang hua qi,yin cheng xing".The dysfunction of the warming and promoting functions of yang qi,and pathological products such as yin cold phlegm stasis accumulate excessively.Therefore,the basic pathogenesis of this disease can be summarized as"yang deficiency as the root,yin as the branch".The treatment is based on warming and tonifying yang qi to eliminate yin,following the principle of the Triple-Jiao order therapy.In the initial stage,the Guizhi method is used to promote the circulation of yang qi in the middle and upper burners,restoring the rise and fall of the qi machine.In the mid-stage,the Sini method is employed to warm,tonify,and consolidate the yuan yang of the lower jiao,cultivating the innate foundation.In the later stage,tonifying the spleen and stomach is used to solidify the source of heavenly transformation.Only through this approach can yang qi resume its normal functions and be firmly secured throughout the body,ultimately achieving the therapeutic goal of"When yin is calm and yang is secure,the spirit-mind attains harmony".
