摘要
目的:探讨益气活络生肌汤联合自体富血小板血浆治疗糖尿病足溃疡(DFU)的临床疗效。方法:前瞻性纳入2021年7月~2022年7月本院收治的106例DFU患者,采用随机数字表法分为对照组和研究组,每组53例。两组均给予常规治疗,对照组给予自体富血小板血浆治疗,研究组给予益气活络生肌汤联合自体富血小板血浆治疗,两组均连续治疗4周。比较两组临床疗效、创面愈合情况、溃疡面积、周围神经传导速度以及不良反应发生情况。结果:研究组治疗总有效率(90.57%)高于对照组(73.58%,P<0.05)。研究组创面愈合时间、足部溃疡肉芽组织出现时间、住院时间均短于对照组,住院费用低于对照组(P<0.05)。治疗1、2、3、4周后,两组溃疡面积均减小,且研究组小于对照组(P<0.05)。治疗4周后,两组尺神经、胫神经、腓神经传导速度均得到改善,且研究组改善程度优于对照组(P<0.05)。治疗期间,两组不良反应总发生率比较无统计学差异(P>0.05)。结论:益气活络生肌汤联合自体富血小板血浆治疗DFU,可提高治疗总有效率,加速创面愈合,减小溃疡面积,并提高周围神经传导速度,其治疗效果优于单用自体富血小板血浆治疗。
Objective:To investigate the clinical efficacy of Yiqi Huoluo Shengji Decoction combined with autologous platelet-rich plasma(PRP)in the treatment of diabetic foot ulcer(DFU).Methods:A total of 106 patients with DFU admitted to our hospital from July 2021 to July 2022 were prospectively enrolled and divided into the control group and the study group by random number table method,with 53 patients in each group.Both groups received conventional treatment,the control group was treated with autologous PRP,while the study group was treated with Yiqi Huoluo Shengji Decoction combined with autologous PRP.Both groups received continuous treatment for 4 weeks.Clinical efficacy,wound healing status,ulcer area,peripheral nerve conduction velocity,and adverse reactions were compared between the two groups.Results:The total effective rate of the study group(90.57%)was higher than that of the control group(73.58%,P<0.05).The study group had shorter wound healing time,earlier appearance time of granulation tissue in foot ulcers,shorter hospital stay,and lower hospitalization costs than the control group(P<0.05).After 1,2,3,and 4 weeks of treatment,the ulcer area decreased in both groups,and the study group had a smaller ulcer area than the control group(P<0.05).After 4 weeks of treatment,the conduction velocities of the ulnar nerve,tibial nerve,and peroneal nerve improved in both groups,and the improvement was more significant in the study group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion:Yiqi Huoluo Shengji Decoction combined with autologous PRP in the treatment of DFU can improve the total effective rate,accelerate wound healing,reduce the ulcer area,and improve peripheral nerve conduction velocity,with a therapeutic effect superior to that of autologous PRP alone.
