摘要
目的探讨囊性部分分化性肾母细胞瘤(CPDN)和囊性肾瘤(CN)的CT及临床表现,结合病理及文献分析,提高影像医师对两种疾病的诊断水平。方法回顾性分析经病理证实的6例CPDN和7例CN的资料。CPDN患者中,男5例,女1例,平均年龄为11.6个月;CN患者中,男4例,女3例,平均年龄为14.6个月。采用Philips 256层螺旋CT进行腹部平扫加三期增强扫描,并参照2019版Bosniak分级和美国儿童肿瘤协作组标准对肾脏囊性病变进行分级和临床分期,以进行肿瘤良恶性评估。结果CPDN组和CN组均表现为囊性肿块,无实性结节。CPDN组较CN组病变体积稍大,囊壁及分隔厚度较厚,囊隔钙化常见,增强后多呈中度强化,Bosniak分级较高。CPDN组Bosniak分级表现为Ⅱ级1例,ⅡF级1例,Ⅲ级4例,CN组Bosniak分级表现为Ⅱ级2例,ⅡF级3例,Ⅲ级2例。CPDN组临床分期Ⅰ期4例,Ⅱ期2例;CN组均为Ⅰ期。结论CT征象可在一定程度上为影像诊断和鉴别诊断提供思路。
Objective To investigate the CT and clinical manifestations of cystic partially differentiated nephroblastoma(CPDN)and cystic nephroma(CN)in children,and to improve radiologists'diagnostic level for these two diseases by combining pathological findings and literature analysis.Methods A retrospective analysis was performed on 6 cases of CPDN and 7 cases of CN confirmed by pathology in our hospital.Among the CPDN patients,there were 5 males and 1 female with an average age of 11.6 months;among the CN patients,there were 4 males and 3 females with an average age of 14.6 months.Phillip 256-slice spiral CT was used for abdominal plain scan and three-phase enhanced scan.The cystic renal lesions were graded and clinically staged with reference to the 2019 version of Bosniak classification and the Children's Oncology Group(COG)criteria to evaluate the benignancy and malignancy of the tumors.Results Both the CPDN group and the CN group presented as cystic masses without solid nodules.Compared with the CN group,the CPDN group had slightly larger lesion volume,thicker cyst walls and septa,more common septal calcification,mostly moderate enhancement,and higher Bosniak grades.In the CPDN group,the Bosniak classification included 1 case of grade Ⅱ,1 case of grade ⅡF,and 4 cases of grade Ⅲ;in the CN group,the Bosniak classification included 2 cases of grade Ⅱ,3 cases of grade ⅡF,and 2 cases of grade Ⅲ.For clinical staging,there were 4 cases of stage Ⅰ and 2 cases of stage Ⅱ in the CPDN group,while all cases in the CN group were stage Ⅰ.Conclusions CT signs can provide certain references for imaging diagnosis and differential diagnosis of CPDN and CN in children.
