基于“诸胀腹大,皆属于热”探讨肝硬化腹水病机与辨治

Discussion on the Pathogenesis and Treatment Based on Syndrome Differentiation for Ascites in Liver Cirrhosis Under the Concept of“All Abdominal Distention is Due to Heat”

摘要 肝硬化腹水为肝脏疾病终末期阶段常见的并发症,其主要表现为腹腔内积液,导致患者腹部膨胀、腹痛等症状,严重影响患者的生活质量和预后,属于中医学“水臌”“臌胀”范畴。聚焦“诸胀腹大,皆属于热”的经典理论,系统阐释肝硬化腹水的中医病机与辨治规律。通过分析湿热瘀毒互结的核心病机,阐释“热”的多元内涵及其在疾病不同阶段的表现,系统阐述肝硬化腹水的辨治策略,提出“清-化-补”递进式辨治体系,构建肝硬化腹水“病-证-治”一体化诊疗方案,为优化中医临床治疗策略,促进中西医结合防治该病提供参考。 As a common complication in the end stage of liver diseases,ascites due to liver cirrhosis is primarily characterized by the accumulation of fluid in the abdominal cavity,leading to symptoms such as abdominal distention and pain,which severely affect the patient's quality of life and prognosis.This condition falls under the categories of“water swelling”and“abdominal distention”in traditional Chinese medicine(TCM).The classic theory“All Abdominal Distention is Due to Heat”systematically explaining the TCM pathogenesis and treatment principles of ascites due to liver cirrhosis.By analyzing the core pathogenesis of damp-heat accumulation and blood stasis,it elucidates the multifaceted connotation of“heat”and its manifestations at different stages of the disease,systematically outlining treatment strategies for ascites due to liver cirrhosis.It aims to construct an integrated diagnosis and treatment system of“disease-syndrome-treatment”for ascites due to liver cirrhosis,providing a reference for optimizing TCM clinical treatment strategies and promoting the integrated prevention and treatment of traditional Chinese and Western medicine.
