基于任务属性的军事训练伤疾病谱系分析

Disease spectrum analysis of military training-related injuries based on task attributes

摘要 目的探讨不同军事作业任务属性与肌肉骨骼运动系统疾病谱系的关联性。方法回顾性分析2023年11月—2025年3月武警福建省总队医院巡诊数据(951人,1008例),按核心任务属性将病例分为执勤组604例(主要承担岗哨监视等静态作业)与训练组404例(主要进行战术演练等动态训练)。依据标准分类疾病,采用卡方检验分析组间疾病谱、致伤机制及解剖部位分布的差异。结果腰肌劳损在两组中均居首位,且执勤组构成比显著高于训练组(19.04%vs.12.87%,P=0.010)。急性损伤中,训练组构成比高于执勤组(P=0.036)。下肢损伤分布中膝关节损伤训练组高于执勤组(37.83%vs.29.13%),足部损伤执勤组高于训练组(20.47%vs.10.87%),差异均有统计学意义(P<0.05)。结论军事任务属性是肌肉骨骼疾病谱系分化的关键因素。长期静态作业易致腰背及足部慢性劳损,而长期动态训练则更易引发下肢(尤其是膝关节)损伤。 Objective To investigate the association between different military operational task attributes and the spectrum of musculoskeletal diseases.Methods A retrospective analysis was conducted on medical patrol data from our hospital between Nov.2023 and Mar.2025,involving 951 individuals with 1008 documented cases.Based on core task attributes,cases were categorized into a duty group(n=604,primarily static tasks such as sentry duty)or a training group(n=404,primarily dynamic training such as tactical exercises).Diseases were classified according to standard criteria.Chi-square test was used to analyze differences in the disease spectrum,injury mechanisms,and affected anatomical sites between the two groups.Results Lumbar muscle strain was the most common injury in both groups,with a significantly higher incidence in the duty group(19.04%vs.12.87%,P=0.010).The proportion of acute injuries was much higher in the training group compared to the duty group(P=0.036).Analysis of lower limb injuries revealed significant differences between the groups:knee injuries were more common in the training group(37.83%vs.29.13%,P<0.05),while foot injuries were more common in the duty group(20.47%vs.10.87%,P<0.05).Conclusion Military task attributes are a critical factor influencing the differentiation of the musculoskeletal disease spectrum.Long-term static duties predispose personnel to chronic lower back and foot injuries,whereas high-intensity dynamic training increases the risk of lower limb injuries,particularly to the knees.
