摘要
目的探究尿可溶性CD163(sCD163)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、视黄醇结合蛋白(RBP)对过敏性紫癜(HSP)患儿肾损伤的诊断效能。方法前瞻性选取2022年1月至2024年8月山东省青岛市胶州中心医院收治的124例HSP患儿作为HSP组,根据是否发生肾损伤分为肾损伤组(42例)及非肾损伤组(82例),另选取130例同期来院进行健康体检的儿童作为对照组。采用Pearson相关性分析尿蛋白/肌酐比值、血清白介素-6(IL-6)与尿sCD163、NGAL、RBP水平的相关性。采用多因素logistic回归分析HSP患儿发生肾损伤的影响因素,并绘制受试者操作特征(ROC)曲线分析尿sCD163、NGAL、RBP对HSP患儿肾损伤的诊断效能。结果与对照组比较,HSP组患儿的尿sCD163、NGAL、RBP水平显著升高,差异有统计学意义(P<0.001)。与非肾损伤组患儿比较,肾损伤组患儿的尿蛋白/肌酐比值、血清IL-6水平及尿sCD163、NGAL、RBP水平均显著升高,差异有统计学意义(P<0.001)。Pearson相关性分析结果显示,HSP患儿的尿蛋白/肌酐比值、血清IL-6水平与尿sCD163、NGAL、RBP水平均呈正相关(r=0.378、0.446、0.456、0.404、0.435、0.538,P<0.001)。多因素logistic回归分析结果显示,尿蛋白/肌酐比值、血清IL-6及尿sCD163、NGAL、RBP均是HSP患儿发生肾损伤的独立危险因素(P<0.05)。尿sCD163、NGAL、RBP联合应用对HSP患儿发生肾损伤的诊断效能高于尿sCD163、NGAL、RBP单独应用(Z=3.182、2.793、3.139,P=0.002、0.005、0.002)。结论HSP患儿尿sCD163、NGAL、RBP水平显著升高,是其发生肾损伤的独立危险因素,尿sCD163、NGAL、RBP联合应用对HSP患儿肾损伤的诊断效能较高。
Objective To explore the diagnostic value of urinary soluble cluster of differentiation 163(sCD163),neutrophil gelatinase-associated lipocalin(NGAL),and retinol-binding protein(RBP)for renal injury in children with Henoch-Schönlein purpura(HSP).Method A total of 124 pediatric HSP patients were enrolled in the HSP group and further divided into a renal injury subgroup(42 cases)and a non-renal injury subgroup(82 cases)based on the occurrence of renal damage.Additionally,130 healthy children undergoing routine physical examinations were selected as the control group.All patients were admitted between January 2022 and August 2024 in Qingdao Jiaozhou Central Hospital.Pearson correlation analysis of urinary protein/creatinine ratio,serum IL-6 and urinary sCD163,NGAL,and RBP levels.Multivariate logistic regression analysis was used to analyze the infl uencing factors.Diagnostic value of receiver operator characteristic(ROC)curve analysis.Result Compared with the control group,the HSP group showed signifi cantly elevated levels of urinary sCD163,NGAL,and RBP,with statistically signifi cant differences(P<0.001).Compared with children in the non-renal injury group,those in the renal injury group exhibited signifi cantly higher levels of urinary protein/creatinine ratio,serum IL-6,and urinary sCD163,NGAL,and RBP,with statistically significant differences(P<0.001).Pearson correlation analysis revealed a positive correlation between the urinary protein/creatinine ratio,serum IL-6 levels,and urinary sCD163,NGAL,and RBP levels in HSP children(r=0.378,0.446,0.456,0.404,0.435,0.538;P<0.001).Multivariate logistic regression analysis indicated that the urinary protein/creatinine ratio,serum IL-6,and urinary sCD163,NGAL,and RBP were independent risk factors for renal injury in HSP children(P<0.05).The combined diagnostic value of urinary sCD163,NGAL,and RBP for renal injury in HSP children was higher than that of individual markers(Z=3.182,2.793,3.139;P=0.002,0.005,0.002).Conclusion The urinary sCD163,NGAL,and RBP in children with HSP are conspicuously elevated,which are infl uencing factors for renal injury in HSP patients.The combination of urinary sCD163,NGAL,and RBP has a high diagnostic value for renal injury in children with HSP.
