扶正补土针灸联合躯干核心肌群训练治疗老年脑卒中恢复期的研究

Efficacy of Fuzheng-Butu Acupuncture Combined With Trunk Core Muscle Training in Elderly Patients During the Convalescent Phase After Stroke

摘要 目的探讨扶正补土针灸联合躯干核心肌群训练治疗老年脑卒中恢复期的疗效。方法选择2022年2月至2024年6月濮阳市安阳地区医院收治的82例老年脑卒中恢复期患者,采用随机数字表法分为研究组和对照组,每组41例。两组均给予抗血小板、改善脑循环、营养神经等常规药物治疗,以及良肢位摆放、关节活动度训练、言语功能训练、吞咽功能训练等常规康复训练。对照组加用躯干核心肌群训练,连续训练6 d后休息1 d。研究组在对照组训练基础上加用扶正补土针灸治疗,连续治疗6 d后休息1 d。两组均治疗4周后判定疗效。比较两组平衡功能[Berg平衡量表(BBS)]、肢体运动功能[简化版Fugl-Meyer运动功能评定量表(FMA)]、美国国立卫生研究院卒中量表(NIHSS)评分、脑动脉血流动力学及神经损伤与修复标志物。结果研究组进步率与对照组比较,差异无统计学意义(P>0.05)。治疗后,两组BBS、FMA评分均升高,且研究组高于对照组,差异有统计学意义(P<0.05);两组NIHSS评分均降低,且研究组低于对照组,差异有统计学意义(P<0.05);两组大脑前动脉、中动脉、后动脉平均血流速度均升高,且研究组高于对照组,差异有统计学意义(P<0.05);两组大脑前动脉、中动脉、后动脉搏动指数(PI)均降低,且研究组低于对照组,差异有统计学意义(P<0.05);两组神经生长因子(NGF)水平均升高,且研究组高于对照组,差异有统计学意义(P<0.05);两组髓鞘碱性蛋白(MBP)和S100钙结合蛋白B(S100B)水平均降低,且研究组低于对照组,差异有统计学意义(P<0.05)。结论扶正补土针灸联合躯干核心肌群训练治疗老年脑卒中恢复期可提升患者平衡功能和肢体运动功能,改善脑动脉血流动力学,减轻神经损伤,促进神经元修复。 Objective To investigate the efficacy of Fuzheng-Butu acupuncture combined with trunk core muscle training in elderly patients during the convalescent phase after stroke.Methods A total of 82 elderly patients in the convalescent phase after stroke admitted to Puyang Anyang Regional Hospital from February 2022 to June 2024 were enrolled and randomly assigned by a random number table method to a study group and a control group(41 patients in each group).Both groups received routine pharmacotherapy.including antiplatelet therapy.medications to improve cerebral circulation.and neurotrophic therapy.as well as conventional rehabilitation training.including proper limb positioning.range-of-motion training.speech function training.and swallowing function training.The control group additionally received trunk core muscle training for 6 consecutive days followed by 1 day of rest.The study group additionally received Fuzheng-Butu acupuncture on the basis of the control group intervention.administered for 6 consecutive days followed by 1 day of rest.Efficacy was evaluated after 4 weeks of treatment in both groups.Balance function[Berg Balance Scale(BBS)].limb motor function[simplified Fugl–Meyer Assessment(FMA)].National Institutes of Health Stroke Scale(NIHSS)score.cerebral arterial hemodynamics.and biomarkers of neural injury and repair were compared between the two groups.Results The improvement rate in the study group did not differ significantly from that in the control group(P>0.05).After treatment.BBS and FMA scores increased in both groups.and both were higher in the study group than in the control group(P<0.05).NIHSS scores decreased in both groups.and the score in the study group was lower than that in the control group(P<0.05).The mean blood flow velocity of the anterior cerebral artery.middle cerebral artery.and posterior cerebral artery increased in both groups and was higher in the study group than in the control group(P<0.05).The pulsatility index(PI)of the anterior cerebral artery.middle cerebral artery.and posterior cerebral artery decreased in both groups and was lower in the study group than in the control group(P<0.05).Nerve growth factor(NGF)levels increased in both groups and were higher in the study group than in the control group(P<0.05).Levels of myelin basic protein(MBP)and S100 calcium-binding protein B(S100B)decreased in both groups and were lower in the study group than in the control group(P<0.05).Conclusion Fuzheng-Butu acupuncture combined with trunk core muscle training can improve balance and limb motor function.optimize cerebral arterial hemodynamics.alleviate neural injury.and promote neuronal repair in elderly patients during the convalescent phase after stroke.
