摘要
目的:运用网状Meta分析方法评价不同中药注射液联合常规西药治疗妊娠期高血压的有效性和安全性。方法:计算机检索PubMed、Cochrane Library、中国生物医学文献服务系统(SinoMed)、Web of Science、EMbase、中国知网(CNKI)、中国生物医学文献数据库、维普和万方数据库中有关中药注射液联合常规西药治疗妊娠期高血压的随机对照试验文献,检索时限为2000年1月1日—2023年12月31日。所有研究人员均独立筛选文献、提取资料、质量评价,对符合质量标准的研究采用Stata 14.0软件进行分析,ROB2进行质量评价。结果:共纳入38项研究,涉及9种干预措施,共3857例受试者。在常规西药的基础上联合脉络宁注射液对提高临床疗效和改善新生儿窒息发生率最有效,联合川芎嗪注射液对改善收缩压最有效,联合丹参川芎嗪注射液对改善舒张压最有效,联合黄芪注射液对改善平均动脉压和24 h尿蛋白最有效,联合参麦注射液对改善产后出血发生率最有效。结论:现有证据表明,涉及9种中药注射液均能改善疗效、收缩压、舒张压、平均动脉压、24 h尿蛋白等结局指标,受纳入研究质量的限制,所得结论仍待多中心、大样本、双盲随机对照试验对结果进行进一步验证。
Objective:To evaluate the efficacy and safety of different traditional Chinese medicine injections combined with conventional Western medicine for treatment of gestational hypertension by the network Meta-analysis.Methods:The randomized controlled trials with traditional Chinese medicine injections and conventional Western medicines for treatment of gestational hypertension were searched from PubMed,Cochrane Library,SinoMed,Web of Science,EMbase,China National Knowledge Infrastructure(CNKI),Chinese Biomedical Literature Database,VIP and WanFang data from January 1,2000 to December 31,2023.All researchers independently conducted literature screening,data extraction,and quality assessment.The literatures that met the quality standards were analyzed with Stata 14.0 software,and quality evaluation was performed with ROB2.Results:A total of 38 literatures were included,involving 9 intervention measures,and a total of 3857 subjects were involved.On the basis of conventional Western medicine,the combination of Mailuoning Injection showed the most significant effect in improving clinical efficacy and reducing the incidence of neonatal asphyxia,Chuanxiongqin Injection was most effective in improving systolic blood pressure,Danshen Chuanxiongqin Injection was most effective in improving diastolic blood pressure,Huangqi Injection was most effective in improving mean arterial pressure and urinary protein,Shenmai Injection was most effective in reducing the incidence of postpartum hemorrhage.Conclusion:The current evidence showed that all 9 types of traditional Chinese medicine injections could improve therapeutic effects,systolic blood pressure,diastolic blood pressure,mean arterial pressure,urine protein and other outcome indicators.Due to limitations in the quality of included literatures,these findings required further validation through multi-center,large-sample,double-blind randomized controlled trials.
