类风湿因子干扰化学发光法致心肌肌钙蛋白I测定中的假阳性结果分析

Analysis of false positive results in the determination of cardiac troponin I by rheumatoid factor interference chemiluminescence

摘要 目的:聚焦于类风湿因子(RF)对化学发光法测定心肌肌钙蛋白I(cTnI)的干扰现象。方法:选取类风湿关节炎患者40例(RF阳性疾病组)、心肌梗死患者40例(心肌损伤组)以及健康体检者40例(正常对照组)作为研究对象,用化学发光法测定cTnI含量,用免疫比浊法测定RF含量,分析RF含量对于cTnI出现假阳性的影响,并提出有效的临床应对策略。结果:RF阳性疾病组、心肌损伤组RF水平均高于正常对照组,差异有统计学意义(P<0.01),但RF阳性疾病组与心肌损伤组比较,差异无统计学意义(P>0.05)。RF阳性疾病组cTnI假阳性率高于心肌损伤组和正常对照组,差异有统计学意义(P<0.05),且RF浓度越高,cTnI假阳性率及检测值越高。经稀释法、聚乙二醇沉淀法处理后,79例cTnI阳性样本中23例恢复正常,假阳性率19.16%。结论:RF可干扰化学发光法检测cTnI并引发假阳性,且与RF浓度呈正相关,临床需结合样本处理、联合检测及临床症状综合判断,降低误诊风险。 Objective:This study focused on the interference of rheumatoid factor(RF)on the determination of cardiac troponin I(cTnI)by chemiluminescence method.Methods:A total of 40 samples from patients with rheumatoid arthritis(RF-positive disease group),40 samples from patients with myocardial infarction myocardial injruy group,and 40 samples from healthy individuals undergoing physical examinations nornal control group were selected as study subjects.The levels of cTnI were measured using chemiluminescence,and the levels of RF were measured using immunoturbidimetry.The study analyzed the impact of RF levels on false-positive results for cTnI and proposed effective clinical management strategies.Results:RF levels were significantly higher in both the RF-positive disease group and the myocardial injury group compared to the normal control group(P<0.01),but no significant difference was observed between these two groups(P>0.05).The false positive rate of cTnI in the RF-positive disease group was higher than that in the myocardial injury group and the normal control group(P<0.05).Furthermore,higher RF concentrations correlated with increased cTnI false-positive rates and detection values.After dilution and polyethylene glycol precipitation,23 of 79 cTnI-positive samples returned to normal,yielding a false-positive rate of 19.16%.Conclusion:RF interferes with chemiluminescence-based cTnI detection,causing false positives that correlate positively with RF concentration.Clinicians should integrate sample processing,combined testing,and clinical symptoms for comprehensive evaluation to reduce misdiagnosis risks.
