摘要
目的:基于“肠肾轴”理论探讨温肾排毒方灌肠联合穴位注射对慢性肾脏病(chronic kidney disease,CKD) 4—5期患者微炎症状态、肾功能的影响。方法:选取2023年1月至2024年12月于郑州人民医院肾内科住院治疗的未透析CKD 4—5期伴恶心呕吐患者64例,采用随机数字表法分为观察组与对照组,每组各32例。两组患者入院后均给予CKD一体化常规基础治疗,对照组另给予甲氧氯普胺注射液肌内注射治疗,观察组另给予温肾排毒方灌肠联合甲氧氯普胺注射液足三里穴位注射治疗。观察两组患者治疗前后超敏C反应蛋白(hs-C reactive protein,hs-CRP)、白细胞介素(interleukin,IL)-6、血红蛋白(haemoglobin,Hb)、白蛋白(albumin,ALB)、尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,Scr)、血磷(phosphorus,P)、全段甲状旁腺激素(intact parathyroid hormone,i PTH)变化情况及临床疗效。结果:观察组有效率为84.4%,对照组有效率为62.5%,观察组有效率高于对照组,差异有统计学意义(P<0.05)。观察组治疗3周后、治疗6周后hs-CRP和IL-6水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组BUN、Scr水平低于对照组,ALB水平高于对照组,差异有统计学意义(P<0.05)。两组治疗后Hb、P及i PTH水平比较,差异无统计学意义(P > 0.05)。结论:温肾排毒方灌肠联合足三里穴位注射能有效抑制CKD 4—5期患者微炎症状态,改善肾功能与营养状况,缓解消化道症状,其作用机制可能与调节“肠肾轴”、通腑泄浊解毒密切相关。
Objective:To explore the effects of Kidney-Warming and Toxin-Expelling Formula for enema combined with acupoint injection on the micro-inflammatory state and renal function in patients with chronic kidney disease(CKD)stage 4-5,based on the"gut-kidney axis"theory.Methods:A total of 64 non-dialysis CKD stage 4-5 patients with nausea and vomiting,hospitalized in the Nephrology Department of People′s Hospital of Zhengzhou from January 2023 to December 2024 were selected and randomly divided into the observation group and the control group using the random number table method,with 32 cases in each group.After admission,both groups first received integrated conventional basic treatment for CKD.Then the control group additionally received intramuscular injection of Metoclopramide Hydrochloride Injection,while the observation group additionally received Kidney-Warming and Toxin-Expelling for enema combined with Metoclopramide Hydrochloride Injection for acupoint injection at Zusanli(ST36).Changes in hypersensitive C-reactive protein(hs-CRP),interleukin(IL)-6,hemoglobin(Hb),albumin(ALB),blood urea nitrogen(BUN),serum creatinine(Scr),phosphorus(P),intact parathyroid hormone(iPTH)before and after treatment,and clinical efficacy were observed in the two groups.Results:The effective rate was 84.4%in the observation group,which was higher than 62.5%in the control group,with a statistically significant difference(P<0.05).After 3 weeks and 6 weeks of treatment,the levels of hs-CRP and IL-6 in the observation group were all lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the levels of BUN and Scr in the observation group were lower than those in the control group,while the ALB level in the observation was higher than that in the control group,with statistically significant differences(P<0.05).However,there were no statistically significant differences in Hb,P,and iPTH levels between the two groups after treatment(P>0.05).Conclusion:Kidney-Warming and Toxin-Expelling Formula for enema combined with acupoint injection at Zusanli(ST 36)can effectively suppress the micro-inflammatory state,improve renal function and nutritional status,and alleviate gastrointestinal symptoms in patients with CKD stage 4-5.Its mechanism of action may be closely related to regulating the"gut-kidney axis"and promoting detoxification by purging the fu-organ turbidity.
