摘要
随着我国下肢动脉疾病患病率持续升高以及腔内治疗技术广泛普及,建立规范、统一的手术质量评价体系已成为提升医疗质量同质化的迫切需求。本共识由国家外周血管介入技术医疗质量控制中心专家委员会下肢动脉疾病专家工作组牵头,基于国内外最新的权威指南与临床研究证据,通过系统文献筛选和多轮专家讨论,构建了一套适用于中国国情的手术质量评价指标体系。该体系共涵盖7项核心质量指标,包括术后住院死亡率、术后住院期间非计划大截肢率、非计划再次干预率、新发心肌梗死和脑梗死发生率、严重出血事件发生率、踝肱指数(ABI)改善率以及术后平均住院天数,并明确了各指标的定义、计算方法和临床意义。本共识旨在推动全国各级医疗机构下肢动脉腔内治疗手术的规范化和标准化,为实现优质医疗资源下沉和手术质量均质化提供科学依据。
With the continuous increase in the prevalence of lower extremity arterial disease in China and the widespread adoption of endovascular therapy,establishing a standardized and unified surgical quality evaluation system has become an urgent requirement for improving the homogenization of medical quality on this disease.This consensus,led by the Expert Working Group on Lower Extremity Arterial Diseases of the Expert Committee of the National Quality Control Center for Peripheral Vascular Interventional Technology,was developed based on the latest domestic and international authoritative guidelines and clinical research evidence.Through systematic literature screening and multiple rounds of expert discussions,a set of surgical quality evaluation indicators tailored to China’s healthcare context was established.The system includes seven core quality indicators:in-hospital mortality after surgery,rate of unplanned major amputations,rate of unplanned repeat interventions,incidence of new-onset myocardial infarction and cerebral infarction,rate of major bleeding events,ankle-brachial index improvement rate,and average length of hospital stay.Definitions,calculation methods,and clinical significance for each indicator are clearly defined and specified.This consensus aims to promote the standardization and normalization of lower extremity endovascular procedures across medical institutions at all medical levels in China,providing a scientific basis for the dissemination of high-quality medical resources and the homogenization of surgical quality on this disease.
