摘要
目的:探讨滋阴活血润燥汤联合火针对神经性皮炎(阴虚血瘀证)患者临床疗效及对T淋巴细胞的影响。方法:本研究收集2022年11月1日至2025年5月31日就诊于宁德人民医院皮肤科门诊,诊断为神经性皮炎(阴虚血瘀证)的患者60例,分为对照组和治疗组(随机数字表法),各30例。对照组服用滋阴活血润燥汤治疗,治疗组患者在服用滋阴活血润燥汤的同时配合火针治疗。治疗4周后,观察两组患者临床疗效之间的差异性,比较治疗前后主客观症状评分、皮肤病生活质量指标调查表(Dermatology Life Quality Index,DLQI)评分、白细胞介素(Interleukin,IL)-6及T细胞亚群水平。结果:治疗后,治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组DLQI评分、IL-6水平较治疗前显著下降(P<0.05),且治疗组的降幅显著大于对照组(P<0.05)。在T淋巴细胞亚群方面CD8^(+)、CD4^(+)、CD4^(+)/CD8^(+)值均较治疗前有所改善,其中治疗组CD4^(+)/CD8^(+)值显著低于对照组(P<0.05),而CD8^(+)、CD4^(+)虽优于对照组,但差异无统计学意义(P>0.05)。结论:滋阴活血润燥汤联合火针治疗神经性皮炎(阴虚血瘀证)具有较好疗效,能改善患者皮损状态,降低机体炎性反应,改善患者免疫功能,值得借鉴与应用。
Objective:To observe the clinical efficacy of the Ziyin Huoxue Runzao decoction(滋阴活血润燥汤)combined with fire needle therapy on patients with neurodermatitis[Yin(阴)-deficiency and blood-stasis syndrome]and explore its regulatory effect on T-cells.Methods:A total of 60 patients with neurodermatitis(Yin-deficiency and blood-stasis syndrome)who visited the Department of Dermatology of Ningde People’s Hospital from November 1,2022,to May 31,2025 were enrolled.They were randomly assigned to the control group and the treatment group,30 cases in each group.The control group received oral administration of the Ziyin Huoxue Runzao decoction,and the treatment group was treated by needle therapy additionally.Both groups were treated for 4 weeks.Clinical efficacy,subjective/objective symptoms scores,DLQI scores,IL-6 levels,and T-cell subsets levels in the two groups were compared before and after treatment.Results:The total effective rate in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the DLQI scores and IL-6 levels were decreased significantly in both groups(P<0.05).Both groups exhibited omelioration in T lymphocyte subsets(CD8^(+),CD4^(+),and CD4^(+)/CD8^(+)value)compared with baseline.The treatment group showed a significantly lower CD4^(+)/CD8^(+)value than the control group(P>0.05),while CD8^(+)and CD4^(+)levels were higher in the treatment group without statistical significance(P<0.05).Conclusion:The Ziyin Huoxue Runzao decoction combined with fire needle therapy demonstrates significant efficacy in treating neurodermatitis with Yin deficiency and blood stasis syndrome.This approach improves skin lesion conditions,mitigates inflammatory responses,and regulates immune function,is worthy of reference and application.
