摘要
目的 探讨规培医师在规范化培训期间的压力来源及其对职业选择的影响,为制订针对性的职业引导与压力缓解策略提供参考。方法 于2024年10月至2025年4月,采用目的抽样法选取杭州市某三级甲等医院16名规培医师进行半结构式访谈,运用Colaizzi七步分析法对访谈资料进行整理与分析,提炼相关主题。结果 共归纳出2个主题和7个亚主题:压力来源(工作负荷压力、学习压力、人际关系压力、经济压力、社会压力),以及压力对职业选择的影响(职业倦怠与职业认同感水平下降、职业转换倾向)。结论 规培医师面临多方面压力源,这些压力显著影响其身心健康与职业选择,部分规培医师表示在持续高压下可能选择转行或转科以缓解压力。建议管理者采取相应措施,优化规培医师的培养环境,促进其职业发展和心理健康。
Objective To explore the sources of standardized training physicians'stress during their standardized training and the impact of these stresses on their career choices,so as to provide references for formulating targeted career guidance and stress relief strategies.Methods From October 2024 to April 2025,16 standardized training physicians from a Grade A tertiary hospital in Hangzhou were selected by purposive sampling for semi-structured interviews.The interview data were organized and analyzed by Colaizzi's seven-step analysis method,and relevant themes were extracted.Results A total of 2 themes and 7 sub-themes were summarized:sources of stress(workload stress,learning stress,interpersonal relationship stress,economic stress,social stress)and the impact of stress on career choices(occupational burnout and decreased professional identity,career change tendency).Conclusion Standardized training physicians face multiple sources of stress,which significantly affect their physical and mental health and career choices.Some standardized training physicians stated that they might choose to change occupations or departments to relieve stress under continuous high pressure.It is suggested that managers take corresponding measures to optimize the training environment for standardized training physicians and promote their career development and mental health.
