摘要
巴元明教授认为慢性肾衰竭(CRF)的病因主要为禀赋不足,劳倦内伤,饮食不节等,核心病机为气阴两虚,水浊内停,脾肾功能失调,治疗当从虚、水、浊论治,从而确立了益气养阴,通利泻浊为关键治法。以自拟精天颗粒为核心方,补泻兼施,润燥并用。临证注重辨兼夹证,在核心方的基础上,灵活化裁,针对湿热酿毒证,加入积雪草、六月雪清热利湿,解毒活血;肾精不固证,加入分心木、补骨脂补肾健脾,固涩精微;瘀阻肾络证,加入桃仁、苏木活血祛瘀,通便消滞,为中医药治疗CRF提供新思路。
Professor BA Yuanming believes that the etiology of chronic renal failure(CRF)mainly includes congenital insufficiency,internal injury due to overexertion,and improper diet.The core pathogenesis is characterized by deficiency of both Qi and Yin,internal retention of water and turbidity,and dysfunction of the spleen and kidneys.Accordingly,treatment should be based on deficiency,water retention,and turbidity,thereby establishing the key therapeutic principles of tonifying Qi and nourishing Yin,as well as promoting diuresis and eliminating turbidity.The self-formulated Jingtian Granules serve as the core prescription,combining tonification and purgation,as well as moistening and drying therapies.In clinical practice,emphasis is placed on the differentiation of concomitant syndromes,with flexible modifications made on the basis of the core formula.For syndromes of dampness-heat generating toxin,Centellae Herba and Serissa japonica are added to clear heat,promote diuresis,remove toxin,and activate blood circulation.For syndromes of unstable kidney essence,Juglans regia and Psoraleae Fructus are added to tonify the kidneys,strengthen the spleen,and consolidate essence.For syndromes of blood stasis obstructing the renal collaterals,Persicae Semen and Sappan Lignum are added to activate blood circulation,remove stasis,promote bowel movement,and relieve stagnation.This approach provides new ideas for the treatment of chronic renal failure with traditional Chinese medicine.
