摘要
目的:探讨阴道镜联合宫颈环形电切术(LEEP)治疗高级别宫颈鳞状上皮内病变(HSIL)的临床效果。方法:选取2022年1月—2024年1月南通市通州区人民医院妇科收治的76例HSIL患者为研究对象,随机分为试验组和对照组,各38例。对照组行冷刀宫颈锥切术,试验组采用阴道镜联合LEEP治疗。比较两组手术效果。结果:试验组手术时间、术后阴道出血时间以及住院时间比对照组短,术中出血量少于对照组(P<0.001)。试验组术后并发症总发生率为2.63%,低于对照组(P<0.001)。试验组复发率低于对照组(χ^(2)=7.056,P<0.001)。术前,两组精神健康、身体健康、社会功能评分比较,无统计学差异(P>0.05);术后1周,两组精神健康、身体健康、社会功能评分提高,且试验组高于对照组(P<0.001)。结论:相较于冷刀宫颈锥切术,阴道镜联合LEEP治疗HSIL的临床效果更佳,可缩短手术时间,减少术中出血量,降低术后并发症发生率,患者恢复更快,术后生活质量更高。
Objective:To investigate the clinical effect of colposcopy combined with loop electrosurgical excision procedure(LEEP)in the treatment of high-grade squamous intraepithelial lesion(HSIL).Methods:A total of 76 patients with HSIL admitted to the Department of Gynecology at Nantong Tongzhou People's Hospital from January 2022 to January 2024 were selected as the study subjects.They were randomly divided into an experimental group and a control group,with 38 patients in each group.The control group underwent cold knife cervical conization surgery,and the experimental group was treated with colposcopy combined with LEEP.The surgical effects were compared between the two groups.Results:Compared with the control group,the experimental group had shorter operation time,postoperative vaginal bleeding time,and length of hospital stay,and less intraoperative blood loss(P<0.001).The total incidence of postoperative complications in the experimental group was 2.63%,which was lower than that in the control group(P<0.001).The recurrence rate in the experimental group was lower than that in the control group(χ^(2)=7.056,P<0.001).Before surgery,there was no statistically significant difference in the scores of mental health,physical health and social function between the two groups(P>0.05).At 1 week after surgery,the scores of mental health,physical health and social function in the two groups increased,and the scores in the experimental group were higher than those in the control group(P<0.001).Conclusion:Compared with cold knife cervical conization,colposcopy combined with LEEP has better clinical effect in the treatment of HSIL,and can shorten the operation time,reduce the amount of intraoperative blood loss,reduce the incidence of postoperative complications,promote a faster recovery of patients and achieve better postoperative quality of life.
