摘要
目的:比较可弯曲负压吸引鞘辅助下的输尿管软镜碎石术与经皮肾镜碎石术(PCNL)治疗上尿路结石的效果。方法:选取2023年6月—2025年6月广东省河源市源城区人民医院收治的120例上尿路结石患者作为研究对象,随机分为试验组(实施可弯曲负压吸引鞘辅助下的输尿管软镜碎石术)和对照组(实施PCNL),各60例。比较两组手术效果。结果:相较于对照组,试验组住院时间更短(P<0.001)。两组结石清除率比较,无统计学差异(P>0.05)。术后1 d,两组白细胞计数、中性粒细胞计数、C反应蛋白水平提高,但试验组低于对照组(P<0.001)。两组结石清除率比较,无统计学差异(P>0.05)。术前,两组白细胞计数、中性粒细胞计数、C反应蛋白水平比较,差异无统计学意义(P>0.05);术后1 d,两组白细胞计数、中性粒细胞计数、C反应蛋白水平提高,但试验组低于对照组(P<0.001)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论:可弯曲负压吸引鞘辅助下的输尿管软镜碎石术与PCNL治疗上尿路结石的效果均理想,可保证结石清除率、治疗安全性,但前者手术创伤小,可促进患者恢复,减轻术后炎性反应。
Objective:To compare the effects of flexible negative pressure suction sheath assisted flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy(PCNL)in treatment of upper urinary tract calculi.Methods:A total of 120 patients with upper urinary tract stones admitted to Yuancheng District People's Hospital in Heyuan City,Guangdong Province from June 2023 to June 2025 were selected as the study subjects and randomly divided into the experimental group(undergoing flexible ureteroscopic lithotripsy assisted by a bendable negative pressure sheath)and the control group(undergoing PCNL),60 cases in each group.The surgical outcomes of the two groups were compared.Results:Compared with the control group,the experimental group had a shorter hospital stay(P<0.001).There was no statistical difference in the stone clearance rate between the two groups(P>0.05).On postoperative day 1,white blood cell count,neutrophil count,and C-reactive protein levels increased in both groups,but the experimental group had lower levels than the control group(P<0.001).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Both flexible negative pressure suction sheath assisted flexible ureteroscopic lithotripsy and PCNL achieve ideal effects in the treatment of upper urinary tract calculi,and can ensure the stone clearance rate and treatment safety.However,the former surgery has less surgical trauma,and can promote the recovery of patients,and relieve postoperative inflammatory response.
