中医药靶向干预PI3K/Akt信号通路治疗糖皮质激素性骨质疏松的研究进展

Research Progress on Traditional Chinese Medicine Targeting the PI3K/Akt Signaling Pathway in the Treatment of Glucocorticoid-Induced Osteoporosis

摘要 糖皮质激素性骨质疏松(GIOP)是临床上常见的继发性病症,其发病率逐年递增。当下临床上针对GIOP的治疗主要采用双膦酸盐及激素替代等疗法,但其诱发的不良反应多见,影响患者生存质量。国内外对GIOP的发生发展及中药干预机制已展开诸多研究,发现部分中药可通过调控磷脂酰肌醇-3-激酶(PI3K)/蛋白激酶B(Akt,又称PKB)相关信号分子及通路,调节成骨与破骨平衡、氧化应激水平及自噬等功能,对GIOP进行防治。本研究综述中医药干预PI3K/Akt信号通路治疗GIOP的进展,总结通过干预该通路达到治疗目的的中药活性成分及组方,旨在为洞悉GIOP发病机制、探寻治疗靶点提供科学依据,同时为临床研究与药物研发开拓新思路。 Glucocorticoid-induced osteoporosis(GIOP)is a common clinical secondary disorder,with its incidence increasing annually.Current clinical management of GIOP primarily relies on therapies such as bisphosphonates and hormone replacement.However,these approaches are frequently associated with adverse effects that impact patients'quality of life.Numerous studies,both domestically and internationally,have explored the pathogenesis of GIOP and the interventional mechanisms of Chinese material medica.It has been discovered that certain Chinese material medica can prevent and treat GIOP by regulating key signaling molecules and pathways,particularly the phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt,also known as PKB)pathway.This regulation influences the balance between osteogenesis and osteoclastogenesis,oxidative stress levels,autophagy,and other related functions.This review summarizes the advancements in Chinese material medica interventions for GIOP via the PI3K/Akt signaling pathway,and consolidates the active components and formulated prescriptions of Chinese material medica that exert therapeutic effects by targeting this pathway.The aim is to provide a scientific basis for understanding the pathogenesis of GIOP and identifying novel therapeutic targets,while also opening new avenues for clinical research and drug development.
