摘要
目的观察裴氏六味夏破汤加同步放化疗治疗中晚期贲门癌患者的临床效果。方法选择2024年9月1日至2025年8月31日我院收治的120例中晚期贲门癌患者,按照随机数字表法分为放化疗组和联合组,各60例。放化疗组采用放化疗同步治疗,联合组在放化疗同步治疗的基础上加用裴氏六味夏破汤治疗。比较两组患者的临床疗效、肿瘤标志物、生存质量、体能状态、中医证候积分、癌因性疲乏评分、免疫功能及不良反应的发生情况。结果治疗后,联合组的疾病控制率和客观缓解率均高于放化疗组(P<0.05);治疗后,联合组的中医证候积分、Piper疲乏量表(PFS)评分以及糖类抗原72-4(CA72-4)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、CD8^(+)水平均低于放化疗组(P<0.05);治疗后,联合组的卡氏功能状态(KPS)评分、生存质量测定量表-100(QOL-100)评分以及CD3^(+)、CD4^(+)水平均高于放化疗组(P<0.05);治疗后,联合组放射性食管炎、胃肠道反应、贫血、白细胞减少、血小板减少、肝肾功能损伤等放化疗后不良反应的发生率均低于放化疗组(P<0.05)。结论在放化疗同步治疗的基础上加用裴氏六味夏破汤治疗中晚期贲门癌患者,能提高放化疗治疗效果,改善肿瘤标志物、生存质量、体能状态、中医证候积分、癌因性疲乏评分、免疫功能等指标,并降低放化疗后不良反应的发生率。
Objective To observe the clinical effect of Pei's Liuwei Xiapo Decoction combined with concurrent chemoradiotherapy in the treatment of patients with advanced cardia cancer.Methods A total of 120 patients with advanced cardia cancer admitted to our hospital from September 1,2024 to August 31,2025 were selected and divided into chemoradiotherapy group and combined group according to the random number table method,with 60 cases in each group.The chemoradiotherapy group was treated with concurrent chemoradiotherapy,and the combined group was treated with Pei's Liuwei Xiapo decoction on the basis of concurrent chemoradiotherapy.The clinical efficacy,tumor markers,quality of life,physical status,traditional Chinese medicine(TCM)symptom score,cancer-related fatigue score,immune function and adverse reactions were compared between the two groups.Results After treatment,the disease control rate and objective response rate of the combined group were higher than those of the chemoradiotherapy group(P<0.05).After treatment,TCM symptom score,Piper fatigue scale(PFS),carbohydrate antigen 72-4(CA72-4),carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 125(CA125)and CD8+in the combined group were lower than those in the chemoradiotherapy group(P<0.05).After treatment,the Karnofsky functional status(KPS)score,quality of life measurement list-100(QOL-100)score and the levels of CD3+and CD4+in the combined group were higher than those in the chemoradiotherapy group(P<0.05).After treatment,the incidence of adverse reactions such as radiation esophagitis,gastrointestinal reaction,anemia,leukopenia,thrombocytopenia,liver and kidney function injury in the combined group was lower than that in the chemoradiotherapy group(P<0.05).Conclusion On the basis of concurrent chemoradiotherapy,Pei's Liuwei Xiapo decoction can improve the therapeutic effect of chemoradiotherapy in patients with advanced cardia cancer,improve tumor markers,quality of life,physical status,TCM symptom score,cancer-related fatigue score,immune function and other indicators,and reduce the incidence of adverse reactions after chemoradiotherapy.
