摘要
目的比较共面与非共面容积弧形调强放射治疗(VMAT)在胸腺瘤术后放疗中的剂量学差异(共面VMAT计划vs.非共面VMAT计划)。方法回顾性收集2023年2月至2024年12月期间于广州医科大学附属第一医院放疗科经病理确诊的15例胸腺瘤术后患者的放疗计划数据,在MONACO计划系统中分别设计共面VMAT与非共面VMAT计划,处方剂量为50 Gy/25次。评估靶区适形指数(CI)、均匀性指数(HI)及危及器官(肺、心脏、脊髓)的受照剂量参数。结果两种计划在靶区CI和HI比较,差异无统计学意义(P>0.05)。非共面VMAT计划显著降低了全肺低剂量照射体积及平均剂量,但在肺20 Gy受照剂量体积百分比(V20)、30 Gy受照剂量体积百分比(V30)及心脏、脊髓受照剂量方面比较,差异无统计学意义(P>0.05)。结论在胸腺瘤VMAT计划中,非共面VMAT计划在减少肺低剂量照射方面优于共面VMAT计划,而对靶区覆盖及其他危及器官保护未见明显优势。
Objective To compare the dosimetric differences between Coplanar and Noncoplanar Volume Modulated Arc Therapy(CO-VMAT vs.NONCO-VMAT)in Postoperative Radiotherapy for Thymoma.Methods A retrospective analysis was conducted on 15 patients who underwent postoperative radiotherapy for thymoma at the First Affiliated Hospital of Guangzhou Medical University from February 2023 to December 2024.Both CO-VMAT and NONCO-VMAT plans were designed in the MONACO planning system,with a prescribed dose of 50 Gy in 25 fractions.Target conformity index(CI),homogeneity index(HI),and radiation dose parameters for organs at risk(lungs,heart,spinal cord)were evaluated.Results There was no statistically significant difference in CI and HI between the two plans in the target area(P>0.05).NONCO-VMAT significantly reduced the whole-lung low-dose irradiation volume and mean dose,but showed no statistically significant differences in the volume fraction receiving≥20 Gy(V20),the percentage volume receiving 30 Gy(V30),and the doses received by the heart and spinal cord.Conclusion In thymoma VMAT planning,NONCO-VMAT demonstrated superiority over CO-VMAT in reducing lung dose,while showing no significant advantage in target volume coverage or protection of other critical organs.
