摘要
目的探讨术中体位护理对行髋关节置换术的高血压患者短期及长期术后恢复的影响。方法收集2021年2月至2024年8月在南京中医药大学附属苏州市中医医院行髋关节置换术的高血压患者116例,采用随机数字表法分为对照组(接受常规护理)和观察组(采用术中体位护理干预),每组各58例。比较两组手术和术后指标、并发症发生情况、血压波动以及术后恢复情况。结果观察组患者手术用时、术后切口愈合时间和术后视觉模拟评分法(VAS)评分分别为(96.24±12.13)min、(5.19±1.29)d和(3.82±1.21)分,优于对照组的(112.36±15.47)min、(7.69±2.02)d和(5.75±1.75)分(均P<0.05)。观察组患者并发症的发生率为6.90%,低于对照组的27.59%;观察组患者术后24 h血压波动幅度为(12.37±3.18)mmHg,低于对照组的(18.83±4.42)mmHg;观察组患者术后7 d、3个月、6个月Barthel指数分别为(84.23±8.22)分、(87.56±6.89)分、(91.35±4.28)分,术后6个月Harris髋关节功能评分为(89.62±5.37)分,均高于对照组的(71.66±6.53)分、(76.32±7.15)分、(82.17±5.64)分和(78.45±6.12)分(均P<0.05)。结论术中体位护理可显著改善高血压患者行髋关节置换术的短期术后恢复情况,降低并发症发生率,且对患者长期关节功能及生活自理能力恢复具有积极作用。
Objective To explore the impact of intraoperative position nursing on the short-term and long-term postoperative recovery of hypertensive patients undergoing hip arthroplasty.Methods A total of 116 hypertensive patients who underwent hip arthroplasty at Suzhou Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from February 2021 to August 2024 were collected.They were randomly divided into a control group(receiving routine nursing)and an observation group(receiving intraoperative position nursing intervention)using the random number table method,with 58 cases in each group.The surgical and postoperative indicators,incidence of complications,blood pressure fluctuations,and postoperative recovery status of the two groups were compared.Results The operation time,postoperative wound healing time,and postoperative visual analogue scale(VAS)score of patients in the observation group were(96.24±12.13)min,(5.19±1.29)d,and(3.82±1.21)points,respectively,which were superior to those of the control group((112.36±15.47)min,(7.69±2.02)d,and(5.75±1.75)points)(all P<0.05).The incidence of complications in the observation group was 6.90%,which was lower than that in the control group(27.59%).The blood pressure fluctuation amplitude of patients in the observation group 24 hours after surgery was(12.37±3.18)mmHg,which was lower than that of the control group((18.83±4.42)mmHg).The Barthel index scores of patients in the observation group 7 days,3 months,and 6 months after surgery were(84.23±8.22)points,(87.56±6.89)points,and(91.35±4.28)points,respectively,and the Harris hip function score 6 months after surgery was(89.62±5.37)points,all of those were higher than those of the control group((71.66±6.53)points,(76.32±7.15)points,(82.17±5.64)points,and(78.45±6.12)points)(all P<0.05).Conclusions Intraoperative position nursing can significantly improve the short-term postoperative recovery of hypertensive patients undergoing hip arthroplasty,reduce the incidence of complications,and have a positive effect on the long-term recovery of joint function and self-care ability.
