摘要
目的探讨中和生化汤+屈螺酮炔雌醇片的治疗策略用于人工流产后子宫复旧不全患者中的有效性。方法选取2023年3月至2025年3月滕州市妇幼保健院收治的80例人工流产后子宫复旧不全患者,按治疗方法不同分为两组,每组40例。单一药物组予屈螺酮炔雌醇片治疗,联合用药组在单一药物组基础上联合中和生化汤治疗。对比两组术后康复、子宫复旧、血流动力学指标、不良反应发生情况及生活质量。结果联合用药组的阴道出血时间为(8.15±1.22)d、月经复潮时间为(25.24±2.07)d,均短于单一药物组的(10.35±1.57)d、(32.56±2.48)d;联合用药组阴道出血量为(49.48±5.96)mL,少于单一药物组的(71.55±7.48)mL;差异均有统计学意义(t=6.998、14.331、14.594,P均<0.05)。治疗7 d后,联合用药组宫腔内残留、宫腔积液少于单一药物组,子宫内膜厚度厚于单一药物组,子宫内膜阻力指数、搏动指数、子宫动脉收缩期峰值速度均低于单一药物组,差异均有统计学意义(P均<0.05)。治疗28 d后,联合用药组生理、心理、社会、环境评分均高于单一药物组,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论中和生化汤联合屈螺酮炔雌醇片可协同减轻人工流产后子宫复旧不全患者的临床症状,加快子宫血流恢复,促进子宫复旧及月经复潮,减少阴道出血及宫腔内残留,提高生活质量,且安全性较高。
Objective To investigate the efficacy of Zhonghe Shenghua Decoction+drospirenone and ethinylestradiol tablets in the treatment of uterine involution after induced abortion.Methods Eighty patients with uterine involution insufficiency after induced abortion admitted to Tengzhou Maternal and Child Health Hospital from March 2023 to March 2025 were selected and divided into two groups according to different treatment methods,with 40 cases in each group.The control group was treated with drospirenone and ethinylestradiol tablets alone,and the observation group was treated with Zhonghe Shenghua Decoction on the basis of the control group.The postoperative rehabilitation,uterine involution,hemodynamic indexes,adverse reactions and quality of life were compared between the two groups.Results The vaginal bleeding time and menstrual return time of the observation group were(8.15±1.22)d and(25.24±2.07)d,which were shorter than(10.35±1.57)d and(32.56±2.48)d of the control group,and the vaginal bleeding volume of the observation group was(49.48±5.96)mL,it was less than(71.55±7.48)mL of the control group,and the differences were statistically significant(t=6.998,14.331,14.594,all P<0.05).After 7 days of treatment,the residual uterine cavity and uterine cavity effusion in the observation group were less than those in the control group,the thickness of endometrium was thicker than that in the control group,the endometrial resistance index,pulsation index and the peak systolic velocity of uterine artery were lower than those in the control group,and the differences were statistically significant(all P<0.05).After 28 days of treatment,the physiological,psychological,social and environmental scores of the observation group were higher than those of the control group,and the differences were statistically significant(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of Zhonghe Shenghua Decoction combined with drospirone and ethinylestradiol tablets can synergically alleviate the clinical symptoms of patients with incomplete uterine atrophy after induced abortion,accelerate the recovery of uterine blood flow,promote uterine atrophy and the resumption of menstruation,reduce vaginal bleeding and residual substances in the uterine cavity,improve the quality of life,and has relatively high safety.
