摘要
目的探讨补中益气汤联合盆底肌康复对产后妇女压力性尿失禁脾肺气虚型患者尿流动力学及预后的效果。方法选择医院2020年1月-2023年1月收治的100例产后压力性尿失禁脾肺气虚型患者作为研究对象,将其随机分为研究组与对照组,每组50例,给予对照组盆底肌康复治疗,研究组在对照组治疗方案的基础上,联合应用补中益气汤治疗,治疗时间为3个月。比较两组治疗疗效、中医证候积分、盆底肌力、尿动力学参数以及不良反应发生情况。结果两组患者治疗后Ⅰ、Ⅱ类肌纤维肌力均明显将治疗前增大,疲劳度治疗前减小,且研究组变化幅度均大于对照组(P<0.05),研究组总有效率为94.00%(47/50),明显高于对照组的80.00%(40/50)(P<0.05),治疗后两组神疲、气短、懒言、自汗、总积分均明显下降,且研究组明显低于对照组(P<0.05),两组治疗后最大膀胱容量、充盈期逼尿肌压、最大尿流率、最大尿道闭合压均较治疗前增大,残余尿量均觉治疗前减小,研究组变化幅度大于对照组(P<0.05),两组治疗期间均未见严重不良反应。结论补中益气汤联合盆底肌康复可明显改善产后压力性尿失禁脾肺气虚型患者尿动力学指标,提高盆底肌力,帮助患者获得更加满意的预后效果,适合推广应用。
Objective To investigate the effect of Buzhong Yiqi Decoction(补中益气汤)combined with pelvic floor muscle rehabilitation on urinary fluidity and prognosis of postpartum women with stress urinary incontinence and spleen-lung Qi deficiency.Methods A total of 100 cases of postpartum pressure urinary incontinence patients with spleen-lung Qi deficiency in the hospital from January 2020 to January 2023 were selected as the research objects.The patients were numbered and divided into study group and control group by random number table,50 cases in each group.The control group given pelvic floor muscle rehabilitation treatment.The study group combined with Buzhong Yiqi Decoction.The treatment time was 3 months.The therapeutic effect,TCM syndrome scores,pelvic floor muscle strength,urodynamic parameters and the occurrence of adverse reactions were compared between the two groups.Results The muscle strength of classⅠandⅡmuscle fibers in both groups increased significantly after treatment,while fatigue decreased and the change range of the study group was greater than that of the control group(P<0.05).The total effective rate of the study group was 94.00%,which was significantly higher than that of the control group(80.00%,P<0.05).After treatment,fatigue,shortness of breath,lazy speech,spontaneous sweating and total scores of both groups decreased significantly,and the scores of the study group were significantly lower than those of the control group(P<0.05).After treatment,the maximum bladder capacity,detrusor pressure,maximum urine flow rate and maximum urethral closure pressure of both groups increased compared with those before treatment,and the residual urine volume decreased in the study group compared with that in the control group(P<0.05).No serious adverse reactions were observed in both groups during treatment.Conclusion Buzhong Yiqi Decoction combined with pelvic floor muscle rehabilitation can significantly improve the urodynamics indexes and the pelvic floor muscle strength and help patients with postpartum stress urinary incontinence get a more satisfactory prognosis,which is suitable for popularization and application.
