摘要
目的探讨针刺腹四针联合口服乳果糖治疗慢传输型便秘(STC)患者的临床效果。方法选取我院2022年1月至2024年12月治疗的60例STC患者,按随机数字表法将其分为对照组(n=30)、治疗组(n=30)。对照组口服乳果糖治疗,治疗组给予针刺腹四针联合口服乳果糖治疗。比较两组的临床疗效、便秘症状积分、完全自发排便(CSBMs)情况、72 h结肠排出率及肛管直肠功能(指标包括肛管静息压、肛管排便压、直肠最大收缩压、初始排便阈值)。结果治疗组的治疗总有效率显著高于对照组(P<0.05)。治疗后,两组的粪便性状、用力排便、排便不尽感及排便频率积分均明显下降,且治疗组低于对照组(P<0.05)。两组治疗后的CSBMs情况均显著改善,且治疗组优于对照组(P<0.05)。两组治疗后的72 h结肠排出率均明显提高,且治疗组高于对照组(P<0.05)。治疗组治疗后的肛管静息压、肛管排便压、初始排便阈值低于对照组,直肠最大收缩压高于对照组(P<0.05)。结论针刺腹四针联合口服乳果糖治疗STC患者的疗效显著,可明显改善患者的便秘症状,具有较高的临床推广价值。
Objective To investigate the clinical effect of abdominal four-needle acupuncture combined with oral lactulose in the treatment of slow transit constipation(STC)patients.Methods Sixty patients with STC treated in our hospital from January 2022 to December 2024 were selected and divided into control group(n=30)and treatment group(n=30)according to the random number table method.The control group was treated with oral lactulose,and the treatment group was treated with abdominal four-needle acupuncture combined with oral lactulose.The clinical efficacy,constipation symptom score,complete spontaneous bowel movements(CSBMs),72 h colon discharge rate and anorectal function(indexes including anal resting pressure,anal defecation pressure,rectal maximum systolic pressure,initial defecation threshold)were compared between the two groups.Results The total effective rate of treatment in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the scores of fecal traits,forced defecation,incomplete defecation and defecation frequency in the two groups were significantly decreased,and those in the treatment group were lower than the control group(P<0.05).The CSBMs of the two groups were significantly improved after treatment,and that in the treatment group was better than the control group(P<0.05).The 72 h colon rate of the two groups after treatment was significantly increased,and that of the treatment group was higher than the control group(P<0.05).After treatment,the anal resting pressure,anal defecation pressure and initial defecation threshold in the treatment group were lower than those in the control group,and the rectal maximum systolic pressure was higher than that in the control group(P<0.05).Conclusion Abdominal four-needle acupuncture combined with oral lactulose is effective in the treatment of STC patients,it can significantly improve the constipation symptoms of patients and has high clinical promotion value.
