功能性磁刺激对膀胱过度活动症疗效的Meta分析

摘要 目的:系统评价功能性磁刺激治疗膀胱过度活动症(OAB)病人的疗效,为临床提供可靠的循证医学依据。方法:系统检索PubMed、Web of Science、EMbase、Cochrane Library、Scopus、维普数据库(VIP)、中国知网(CNKI)、中国生物医学文献数据库(CBM)、万方数据库(WanFang Data)中关于功能性磁刺激干预OAB病人的随机对照试验。检索时限为建库至2025年1月。2名研究者独立完成文献筛选、数据提取,采用Cochrane手册5.1.0推荐的工具进行偏倚风险评估。采用RevMan 5.4软件进行Meta分析。结果:共纳入11篇文献,涉及765例病人。Meta分析结果显示,功能性磁刺激治疗能降低OAB病人24 h排尿次数[MD=-2.33,95%CI(-2.98,-1.68),P<0.000 01],增加单次排尿量[MD=21.47,95%CI(17.23,25.72),P<0.000 01],降低膀胱过度活动症症状评分(OABSS)[MD=-1.44,95%CI(-2.10,-0.77),P<0.000 1],提高初始尿意膀胱容量[MD=14.45,95%CI(11.56,17.33),P<0.000 01]。结论:功能性磁刺激可有效改善OAB病人的排尿功能及症状,但现有研究存在局限,需更多高质量随机对照试验进一步验证其疗效及安全性。 Objective:To systematically evaluate the efficacy of functional magnetic stimulation in the treatment of patients with overactive bladder(OAB)and provide reliable evidence-based medical evidence for clinical practice.Methods:Randomized controlled trials(RCTs)on magnetic stimulation intervention in patients with OAB were retrieved from PubMed,Web of Science,EMbase,Cochrane Library,Scopus,VIP,CNKI,CBM,and WanFang Data.The retrieval period was from the establishment of the database to January 2025.Two researchers independently completed literature screening and data extraction.The tools recommended in Cochrane Manual 5.1.0 were adopted for risk assessment of bias.RevMan 5.4 software was adopted for Meta-analysis.Results:A total of 11articles were included,involving 765 patients.The results of the Meta-analysis showed that functional magnetic stimulation therapy could reduce the 24 hour urination frequency(MD=—2.33,95%CI—2.98-—1.68,P<0.00001)of OAB patients,increase the single urination volume(MD=21.47,95%CI 17.23-25.72,P<0.00001),lower the OABSS score(MD=-1.44,95%CI-2.10--0.77,P<0.0001),and enhance the initial bladder capacity(MD=14.45,95%CI 11.56-17.33,P<0.00001).Conclusion:Functional magnetic stimulation could effectively improve the urination function and symptoms of patients with OAB.However,existing studies have limitations,and more highquality RCTs are needed to further verify its efficacy and safety.
