摘要
目的探讨泛凋亡在肿瘤治疗中的调控机制及其在化疗增敏、放疗协同和免疫治疗增效等方面的应用潜力。方法以“PANoptosis,programmed cell death,chemotherapy,immunotherapy,nanotherapy”为英文关键词,系统检索PubMed和Web of Science数据库1986-01-01-2025-07-31发表的相关文献。纳入标准:(1)聚焦泛凋亡分子机制的基础与临床研究;(2)探讨泛凋亡在肿瘤治疗中应用价值的研究。排除数据不明、重复及存在争议的文献。最终纳入文献71篇。结果泛凋亡是一种整合焦亡、凋亡和坏死性凋亡的炎症性程序性细胞死亡方式,在癌症治疗领域显示出重要的应用潜力。在化疗中,泛凋亡可逆转肿瘤耐药性并增强药物敏感性;在放疗中,它能提高肿瘤细胞的放射敏感性并克服上皮-间质转化(EMT)相关的抵抗机制;在免疫治疗方面,泛凋亡有助于促进肿瘤抗原暴露和免疫细胞浸润,从而增强免疫检查点抑制剂的疗效。此外,泛凋亡为靶向治疗提供了新的作用靶点,并可协同增强纳米药物的治疗效果。然而,泛凋亡在正常组织中的异常激活可能引发过度炎症反应和组织损伤,因此其临床应用仍面临安全性方面的挑战,需进一步深入研究。结论泛凋亡作为肿瘤治疗的新靶点,对优化现有治疗方案具有重要价值。未来研究应着重阐明泛凋亡在不同肿瘤中的特异性调控网络,并开发个体化联合治疗策略。
Objective To investigate the regulatory mechanisms of PANoptosis in cancer therapy and its potential applica-tions in chemotherapy sensitization,radiotherapy synergy,and immunotherapy enhancement.Methods Using the key-words"PANoptosis,programmed cell death,chemotherapy,immunotherapy,nanotherapy",a systematic search was conducted in PubMed and Web of Science for literature published from January 1,1986,to July 31,2025.Inclusion crite-ria:(1)basic and clinical studies focusing on the molecular mechanisms of PANoptosis;(2)studies exploring the applica-tion value of PANoptosis in cancer treatment.Literature with unclear data,duplication,or controversial findings was ex-cluded.A total of 71 articles were included.Results PANoptosis is an inflammatory programmed cell death pathway that integrates pyroptosis,apoptosis,and necroptosis,demonstrating significant potential in cancer treatment.In chemothera-py,it reverses tumor resistance and enhances drug sensitivity;in radiotherapy,it improves radiosensitivity and counter-acts resistance mechanisms related to epithelial-mesenchymal transition(EMT);in immunotherapy,it promotes tumor an-tigen exposure and immune cell infiltration,thereby improving the efficacy of immune checkpoint inhibitors.Additionally,PANoptosis provides new targets for targeted therapy and synergistically enhances the effects of nanotherapy.However,aberrant activation of PANoptosis in normal tissues may trigger excessive inflammatory responses and tissue damage,in-dicating that its clinical application still faces safety challenges,necessitating further in-depth research.Conclusions As a novel target in cancer therapy,PANoptosis holds great value for optimizing existing treatment strategies.Future research should focus on elucidating the specific regulatory networks of PANoptosis in different tumors and developing personalized combination therapeutic approaches.
