摘要
过敏性紫癜是一种儿童时期常见的一种自身免疫疾病,异常沉积于血管内皮细胞IgA是其主要的病理特点,由此免疫复合物沉积导致自身免疫异常激活,进而引起全身血管免疫炎症性损伤,引起出血是肾单位由毛细血管迂曲围绕的血管球形成,沉积的免疫复合物作为免疫原性物质再度激活自身免疫反应,造成肾单位损伤,进而出现血尿、蛋白尿、进行性肾功能不全。中医学将其归纳为“葡萄疫”“紫斑”“紫癜风”等范畴。“阴络伤则血内溢”理论源于《灵枢·百病始生》,其指出体内血络受损则出现血溢脉络之外的病理征象,与过敏性紫癜的临床表现不谋而合。辽宁中医药大学附属医院儿科以“清润疗法”治疗小儿病,从清透血热、宁润血络角度入手,已形成特色诊疗思路,所创立经验方已应用临床多年,疗效确切。文章总结丰富辽派中医清润疗法的证治内涵,以期传承创新清润流派的思想理念,为治疗紫癜性肾炎提供中医理论基础。
Henoch-Schonlein purpura is a common autoimmune disease in childhood,and its abnormal deposition in vascular endothelial cells IgA is its main pathological feature,and the deposition of immune complexes leads to abnormal activation of autoimmunity,which in turn causes systemic vascular immune inflammatory damage,and hemorrhage is the main pathological process.Because the nephron is formed by the vascular spheres surrounded by capillary tortuosity,the deposited immune complexes act as immunogenic substances to reactivate the autoimmune response,resulting in nephron damage,followed by hematuria,proteinuria,and progressive renal insufficiency.Traditional Chinese medicine summarizes it as:“grape disease”,“purple spots”,“purpura”and other categories.The theory of“blood overflow due to injury to the yin veins"originates from"Lingshu-The Origin of a Hundred Diseases”,which points out that if the blood veins are damaged in the body,pathological signs outside the blood overflow veins will appear,which coincides with the clinical manifestations of Henoch-Schonlein purpura.The Department of Pediatrics of Traditional Chinese Medicine in Liaoning Province treats pediatric diseases with“Qingrun Therapy”,starting from the perspective of clearing blood fever and Ningrun blood veins,and has formed a characteristic diagnosis and treatment idea,and the established experience formula has been applied in clinical practice for many years,and the effect is certain.This paper enriches the connotation of the syndrome and treatment of the Qingrun Therapy of the Liao School of Traditional Chinese Medicine,in order to inherit and innovate the ideological concept of the Qingrun School and provide a theoretical basis for the treatment of purpuric nephritis.
