摘要
目的本研究旨在探讨DNA损伤诱导凋亡抑制因子(DDIAS)对川崎病(KD)的诊断价值,为临床诊疗提供参考。方法选取2017年3月-2019年12月西安交通大学附属儿童医院就诊的96例川崎病儿童作为研究对象,为KD组,对照组为30名同期体检的健康儿童。通过酶联免疫吸附测定(ELISA)试剂盒检测两组儿童外周血中DDIAS、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平,采用受试者工作特征(ROC)曲线评价各因子对KD的诊断价值。结果KD组患儿外周血中DDIAS、IL-6和TNF-α水平显著高于对照组,差异均有统计学意义(Z=-4.125、-6.713、-5.587,P均<0.001),且经治疗后其表达水平均显著降低(Z=-6.900、-6.740、-8.487,P均<0.001)。外周血DDIAS、IL-6和TNF-α三者联合诊断KD的ROC曲线下面积(AUC)最大(AUC=0.977,P<0.001),高于单一因素DDIAS(AUC=0.747,P<0.001)、IL-6(AUC=0.902,P<0.001)和TNF-α(AUC=0.835,P<0.001),外周血DDIAS诊断KD的敏感性和特异性分别为65.62%和80.65%。结论KD组患儿外周血中DDIAS、IL-6和TNF-α水平均升高。DDIAS对KD有较高的诊断价值,并且外周血DDIAS、IL-6和TNF-α联合诊断KD的价值高于单独诊断。
Objective This study aims to explore the diagnostic value of DNA damage-induced apoptosis suppressor(DDIAS)for Kawasaki disease(KD),provide reference for clinical diagnosis and treatment.Methods A total of 96 KD children who visited the Xi'an Children's Hospital from March 2017 to December 2019 were selected as the KD group.The control group was 30 healthy children who underwent physical examination during the same period.The levels of DDIAS,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in peripheral blood of children in the two groups were detected by enzyme-linked immunosorbent assay(ELISA)kit.The receiver operating characteristic curve(ROC)was used to evaluate the diagnostic value of each factor for KD.Results The levels of DDIAS,IL-6 and TNF-αin peripheral blood of children in the KD group were significantly higher than those in the control group,and the differences were statistically significant(Z=-4.125,-6.713,-5.587;all P<0.001),and their expression levels were significantly reduced after treatment(Z=-6.900,-6.740,-8.487;all P<0.001).The area under the ROC curve(AUC)of the combined diagnosis of KD by DDIAS,IL-6 and TNF-αin peripheral blood was the largest(AUC=0.977,P<0.001),which was higher than that of single factor DDIAS(AUC=0.747,P<0.001),IL-6(AUC=0.902,P<0.001)and TNF-α(AUC=0.835,P<0.001).The sensitivity and specificity of peripheral blood DDIAS in diagnosing KD were 65.62%and 80.65%,respectively.Conclusions The levels of DDIAS,IL-6,and TNF-αin the peripheral blood of children in the KD group were all elevated.DDIAS had a high diagnostic value for KD,and the combined diagnostic value of DDIAS,IL-6,and TNF-αin peripheral blood was higher than that of each of them alone.
