摘要
目的:比较阵发性心房颤动和持续性心房颤动患者系统免疫炎症指数、系统炎症反应指数水平以及心脏结构和功能变化。方法:选择2022年1月—2023年12月石河子大学第一附属医院住院治疗的房颤患者290例,其中阵发性房颤181例、持续性房颤109例,另设无房颤患者290例作为对照组。详细记录各组患者基线资料、全身免疫炎症指数、系统炎症反应指数以及超声心动图测量数据。结果:3组系统免疫炎症指数、系统炎症反应指数水平差异无统计学意义,持续性房颤组左心房直径平均水平明显高于阵发性房颤组(P<0.001),且这2组左心房直径平均水平均明显高于对照组(P<0.001)。多因素Logistic回归分析显示,尿酸(OR:1.005,95%CI:1.001~1.009)、心率(OR:1.024,95%CI:1.004~1.043)和左心房直径(OR:1.286,95%CI:1.199~1.379)是持续性房颤的独立危险因素。结论:尿酸、心率、左心房直径的变化与房颤持续时间紧密相关,可能在房颤的发展中起着重要作用。
Objective:To compare systemic immune-inflammation index(SII),systemic inflammatory response index(SIRI),and changes in cardiac structure and function between patients with paroxysmal atrial fibrillation and persistent atrial fibrillation.Methods:A total of 290 patients with atrial fibrillation hospitalized at the First Affiliated Hospital of Shihezi University from January 2022 to December 2023 were enrolled,including 181 patients with paroxysmal atrial fibrillation and 109 patients with persistent atrial fibrillation.Additionally,290 patients without atrial fibrillation were included as a control group.Baseline clinical data,SII and SIRI values,and echocardiographic parameters were collected and analyzed.Results:No significant differences were observed in SII or SIRI levels among the three groups.The mean left atrial diameter in the persistent atrial fibrillation group was significantly larger than that in the paroxysmal atrial fibrillation group(P<0.001),and both were significantly greater than that in the control group(P<0.001).Multivariate logistic regression analysis identified serum uric acid(OR=1.005,95%CI:1.001~1.009),heart rate(OR=1.024,95%CI:1.004~1.043),and left atrial diameter(OR=1.286,95%CI:1.199~1.379)as independent risk factors for persistent atrial fibrillation.Conclusion:Changes in serum uric acid,heart rate,and left atrial diameter are closely associated with the duration of atrial fibrillation and may play important roles in its progression.
