摘要
目的评价天麻钩藤饮加减方联合针刺治疗早期帕金森病(PD)肝肾不足证患者运动功能、炎症因子和谷氨酸(Glu)、γ-氨基丁酸(GABA)、尿酸(UA)等生化指标的影响。方法选取符合纳入标准的早期帕金森病肝肾不足证患者,依据随机数字表随机分为对照组和观察组,每组各60例。对照组给予多巴丝肼片口服治疗,观察组在对照组治疗基础上给予天麻钩藤饮加减方联合针刺治疗,两组均治疗8周。比较两组治疗前后中医症状积分、UPDRS-Ⅲ、NMSS、HAMA、HAMD、PDSS评分和TNF-α、IL-1β、IL-6、Glu、Hcy、GABA、UA含量的变化情况,并对比临床疗效。结果观察组临床总有效率91.67%(55/60),明显优于对照组临床总有效率78.33%(47/60)(P<0.05)。治疗后,两组中医症状积分、UPDRS-Ⅲ、NMSS、HAMA、HAMD评分和TNF-α、IL-1β、IL-6、Glu、Hcy含量均较治疗前明显降低,GABA、UA水平和PDSS评分均较治疗前明显升高(P<0.05)。与对照组治疗后比较,观察组上述指标改善明显,差异有统计学意义(P<0.05)。结论天麻钩藤饮加减方联合针刺可有效改善早期帕金森病肝肾不足证患者的运动与非运动症状,其作用机制可能与降低炎症反应,调节Glu、GABA等神经递质水平有关。
Objective To evaluate the effects of modified Tianma Gouteng Decoction combined with acupuncture on motor function,inflammatory factor and biochemical indicators such as glutamate(Glu),γ-aminobutyric acid(GABA)and uric acid(UA)in patients with early-stage Parkinson's disease(PD)of liver-kidney deficiency pattern.Methods Patients with early-stage PD of liver-kidney deficiency pattern who met the inclusion criteria were divided into a control group and an observation group according to the random number table,with 60 cases in each group.The control group was treated with oral levodopa and benserazide tablets,while the observation group was treated with modified Tianma Gouteng Decoction combined with acupuncture on the basis of the treatment for the control group.Both groups received treatment for 8 weeks.Before and after treatment,the changes in TCM symptom scores,UPDRS-Ⅲ(Unified Parkinson's Disease Rating Scale PartⅢ),NMSS(Non-Motor Symptoms Scale),HAMA(Hamilton Anxiety Rating Scale),HAMD(Hamilton Depression Rating Scale),PDSS(Parkinson's Disease Sleep Scale)scores,and the levels of TNF-α(Tumor Necrosis Factor-α),IL-1β(Interleukin-1β),IL-6(Interleukin-6),Glu,Hcy(Homocysteine),GABA,and UA(Uric Acid)were compared between the two groups,as well as the clinical efficacy.Results The total clinical effective rate of the observation group was 91.67%(55/60),which was significantly higher than that of the control group,78.33%(47/60)(P<0.05).After treatment,compared with the pre-treatment,both groups showed significant decreases in TCM symptom scores,UPDRS-Ⅲ,NMSS,HAMA,HAMD scores,and the contents of TNF-α,IL-1β,IL-6,Glu,and Hcy,as well as significant increases in GABA,UA levels,and PDSS scores(P<0.05).Compared with the control group after treatment,the above indicators in the observation group were improved more significantly,with statistically significant differences(P<0.05).Conclusion Modified Tianma Gouteng Decoction combined with acupuncture can effectively improve the motor and non-motor symptoms of patients with early-stage PD of liver-kidney deficiency pattern,and its mechanism may be related to reduce the inflammatory response,regulate the levels of neurotransmitters such as Glu and GABA.
