基于TOE模型的数字技术赋能基层医疗卫生机构发展动阻力研究

摘要 目的 探析数字技术赋能基层医疗卫生机构发展中存在的动力和阻力。方法 基于TOE模型,对文献分析与关键知情人访谈得到的资料开展主题编码,从技术、组织、环境3个维度系统梳理数字技术赋能基层医疗卫生机构发展中的动力阻力因素。结果 数字技术赋能基层医疗卫生机构发展最主要的技术推动力为数字信息技术的快速发展,主要的组织推动力为政府的鼓励性政策,环境推动力为患者健康需求的日益多样化。数字技术赋能基层医疗卫生机构发展面临的主要技术阻力包括信息孤岛和信息安全隐患,主要的组织阻力包括政策系统不健全、财政支持不够、基层医务人员数字能力不足等,主要环境阻力是患者的认可度不高。结论 数字技术赋能基层医疗卫生机构发展面临的阻力较多而推动力较少,各方要强化数字技术整合应用,建立政策-资金-人才多维度协同路径,提升患者对基层医疗服务数字化的认可度。 Objective Based on the TOE model,to analyze the driving and restraining forces i in the digital development of primary healthcare institutions.Methods Based on the TOE model,the data obtained from literature analysis and key informant interviews were subject coded,and the dynamic resistance factors in the development of digital technology-enabled primary health care institutions were systematically sorted out from three dimensions of technology,organization and environment.Results The main technical driving force for the development of primary health care institutions enabled by digital technology was the rapid development of digital information technology,the main organizational driving force was the government’s incentive policies,and the environmental driving force was the increasing diversification of patients’health needs.The main technical obstacles to the development of digital technology-enabled primary medical institutions include information islands and information security risks,the main organizational obstacles include imperfect policy system,inadequate financial support,inadequate digital capacity of primary medical staff,and the main environmental obstacles are the low recognition of patients.Conclusion The development of primary health care institutions enabled by digital technology faces more resistance and less driving force,so all parties should strengthen the integration and application of digital technology,enhance the multidimensional collaborative path of policy-fund-talent,and enhance patients’recognition of primary health care digitalization.
