摘要
目的探讨输尿管上段复杂性结石(CUU)中输尿管软镜碎石取石术(FURL)与超细经皮肾镜碎石取石术(UMP)的治疗效果。方法对医院2023年1月至2024年12月98例CUU患者的临床资料进行回顾性分析,根据手术方法分为FURL组(42例)及UMP组(56例)。对比手术指标、疼痛程度及来源、炎症因子、并发症。结果相比于FURL组,UMP组术中出血量增加、手术时间缩短、结石清除率升高、住院时间延长(P<0.05);术后1 d及3 d,FURL组及UMP组视觉模拟评分(VAS)比较,差异无统计学意义(P>0.05);2组疼痛来源比较,差异无统计学意义(P>0.05);相较于FURL组,UMP组术后1 d、3 d及7 d血清白细胞介素-8、白细胞介素-6水平均降低(P<0.05);FURL组及UMP组并发症差异无统计学意义(P>0.05);FURL组及UMP组晚期并发症发生率比较,差异无统计学意义(P>0.05)。结论FURL及UMP治疗CUU各有特点,但UMP的手术时间短、结石清除率高,且术后炎症反应较轻。
Objective To explore the curative effect of flexible ureteroscopic lithotripsy(FURL)and ultra-mini-percutaneous nephrolithotomy(UMP)in complex upper ureteral stone(CUU).Methods A retrospective analysis was performed on the clinical data of 98 patients with CUU in the hospital between January 2023 and December 2024.According to different surgical methods,patients were divided into the FURL group(n=42)and the UMP group(n=56).The surgical indexes,pain degree and sources,inflammatory factors and complications in the two groups were compared.Results Compared with the FURL group,intraoperative blood loss was increased,operation time was shortened,stone clearance rate was increased,and length of hospital stay was prolonged in the UMP group(P<0.05).At 1d and 3 d after surgery,there was no significant difference in visual analogue scale(VAS)scores between the two groups(P>0.05).There was no significant difference in pain sources between the two groups(P>0.05).Compared with the FURL group at 1 d,3d and 7 d after surgery,levels of serum interleukin-8 and interleukin-6 were decreased in the UMP group(P<0.05).There was no significant difference in complications between the two groups(P>0.05).There was no significant difference in the incidence of late complications between the two groups(P>0.05).Conclusion Both FURL and UMP have their own characteristics in the treatment of CUU.However,UMP has shorter operation time,higher stone clearance rate and milder postoperative inflammatory response.
