摘要
目的 整合接纳与承诺疗法(acceptance and commitment therapy,ACT)在癌症患者中的应用现状,明确其干预策略、实施特征及效果。方法 基于JBI范围综述框架,检索中国知网、中国生物医学文献数据库、万方、维普、PubMed、Web of science、Embase、PsycINFO、The Cochrane Library等中英文数据库,并补充手工检索相关期刊与会议论文,时限为建库至2025年4月30日,提取纳入研究的干预特征及结局指标。结果 共纳入25篇文献,涵盖乳腺癌、肺癌等多种癌症类型。干预以护士主导的多学科团队实施,整合正念、隐喻等核心技术,联合虚拟现实等创新形式(频次:6~10次/周;时长:25~120 min/次)。ACT可提升心理灵活性、减轻负性情绪、改善生活质量,但对罕见癌种及老年患者研究不足。结论 ACT在癌症患者中应用效果显著,可适应不同癌症类型及疾病阶段,未来需解决干预方案异质性大、特殊人群研究薄弱等问题,并开展多中心研究,拓展应用范围,提升其临床转化价值。
Objective To integrate the current application status of Acceptance and Commitment Therapy(ACT)in cancer patients,clarifying its intervention strategies,implementation characteristics,and effectiveness.Methods Based on the JBI scoping review framework,a search was conducted in Chinese and English databases including CNKI,CBM,Wanfang,VIP,PubMed,Web of Science,Embase,PsycINFO,and The Cochrane Library,with supplementary manual searches of relevant journals and conference proceedings.The search timeframe was from the inception of each database to April 30,2025.Information regarding the intervention characteristics and outcome indicators of the included studies were extracted.Results A total of 25 articles were included,covering various cancer types such as breast cancer and lung cancer.Interventions were primarily delivered by nurse-led multidisciplinary teams,integrating core ACT techniques like mindfulness and metaphor,and combined with innovative formats such as virtual reality(frequency:610 session/week;duration:25120 min/session).Results suggested that ACT can enhance psychological flexibility,alleviate negative emotions,and improve quality of life.However,there is a lack of sufficient research on rare cancers and elderly patients.Conclusion ACT demonstrates significant effectiveness in cancer patients and can be adapted to different cancer types and disease stages.Future research needs to address issues such as high heterogeneity in intervention protocols and insufficient studies on special populations.Multicenter studies should be conducted to expand it application scope and enhance its clinical translational value.
