摘要
目的 分析间歇充气加压泵在全髋关节置换术后深静脉血栓防控中的应用价值。方法 选取全髋关节置换术后患者80例为研究对象,采用分层区组随机化法分为常规组(40例,常规预防措施干预)和研究组(40例,常规预防措施+间歇充气加压泵干预)。比较两组凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fib)]、深静脉血栓发生情况及生活质量评分。结果 干预后,研究组PT(16.12±2.87)s、APTT(27.21±3.11)s均较常规组的(10.33±2.71)、(23.10±2.91)s更长, Fib(2.50±0.22)g/L较常规组的(3.42±0.18)g/L更低(P<0.05)。与常规组的20.00%比较,研究组深静脉血栓发生率2.50%更低(P<0.05)。干预后与常规组的(70.33±2.71)、(73.10±2.91)、(62.50±2.42)、(80.27±2.93)分比较,研究组心理功能评分(89.12±3.87)分、躯体功能评分(82.21±3.11)分、物质生活评分(71.42±2.98)分、社会功能评分(91.14±3.75)分均更高(P<0.05)。结论 在全髋关节置换术后深静脉血栓防控中实施间歇充气加压泵进行干预,对降低深静脉血栓发生率具有积极影响,并可有效改善凝血功能,提升患者生活质量。
Objective To analyze the practical value of intermittent pneumatic compression pump in the prevention and control of deep vein thrombosis(DVT)after total hip arthroplasty(THA).Methods A total of 80 patients after total hip arthroplasty were selected as research subjects.They were divided into the conventional group(40 cases,basic preventive measures)and the study group(40 cases,basic prevention+intermittent pneumatic compression pump intervention)using stratified block randomization.The coagulation function indexes[prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(Fib)],deep vein thrombosis,and quality of life score were compared between the two groups.Results After the intervention,the study group had PT of(16.12±2.87)s and APTT of(27.21±3.11)s,which were longer than those in the conventional group[(10.33±2.71)and(23.10±2.91)s];Fib(2.50±0.22)g/L was lower than the conventional group's(3.42±0.18)g/L(P<0.05).Compared with 20.00%in the conventional group,the incidence of deep vein thrombosis in the study group was lower at 2.50%(P<0.05).Compared with the conventional group[(70.33±2.71),(73.10±2.91),(62.50±2.42),and(80.27±2.93)points],the scores of psychological function(89.12±3.87)points,physical function(82.21±3.11)points,material life(71.42±2.98)points and social function(91.14±3.75)points were higher in the study group after intervention(P<0.05).Conclusion The application of intermittent pneumatic compression pump in the prevention and control of deep vein thrombosis after total hip arthroplasty has a positive impact on reducing the incidence of deep vein thrombosis,and may effectively improve coagulation function and enhance the quality of life of patients.
