摘要
目的探讨一站式CT血管造影(CTA)-CT灌注成像(CTP)对急性缺血性脑梗死(AIS)的侧支循环的评估价值。方法分析2022年8月至2024年8月在本院进行诊治的85例AIS患者,所有患者都给予一站式CTA-CTP检查,记录成像参数与判断侧支循环状况,同时给予数字减影血管造影(DSA)检查进行侧支循环评级判定。结果在85例患者中,DSA检查判断为侧支循环良好48例,占比56%。良好组的NIHSS评分、发病时间与不足组对比差异有统计学意义(P<0.05)。良好组的平均通过时间(MTT)低于不足组,脑血流量(CBF)、脑血容量(CBV)高于不足组,2组对比差异有统计学意义(P<0.05)。Spearsman分析显示NIHSS评分、发病时间与MTT、CBV、CBF存在相关性(P<0.05)。一站式CTA-CTP检查判断为侧支循环良好46例,对AIS的侧支循环的诊断灵敏度、特异度与准确度分别为96%、100%、98%。结论一站式CTA-CTP技术为急性缺血性脑梗死侧支循环的评估提供了高效、准确的手段,同时能够反映患者的临床特征,具有重要的临床应用前景。
Objective To evaluate the value of one-stop CT angiography(CTA)and CT perfusion imaging(CTP)in the assessment of collateral circulation in acute ischemic cerebral infarction(AIS).Methods A total of 85 AIS patients treated in our hospital from August 2022 to August 2024 were analyzed.All patients were given a one-stop CTA-CTP examination,imaging parameters were recorded and the status of collateral circulation was determined.DSA examination was given for collateral circulation rating.Results Among the 85 patients,48 cases(56%)were judged to have good collateral circulation by DSA examination.There were significant differences in NIHSS score and onset time between the good group and poor group(P<0.05).The mean passage time(MTT)of the good group was lower than that of the deficient group,and the cerebral blood flow(CBF)and cerebral blood volume(CBV)were higher than that of the deficient group,and the difference between the two groups was statistically significant(P<0.05).Spearsman analysis showed that NIHSS score and onset time were correlated with MTT,CBV and CBF(P<0.05).The sensitivity,specificity and accuracy of the diagnosis of AIS collateral circulation were 96%,100%and 98%,respectively,in 46 cases with good collateral circulation by CTA-CTP.Conclusion The one-stop CTA-CTP technique provides an efficient and accurate means for the assessment of collateral circulation in acute ischemic cerebral infarction,and can reflect the clinical characteristics of patients,which has an important clinical application prospect.
