摘要
目的探讨3.0T动态对比增强磁共振成像(DCE-MRI)在直肠癌术前分期中的诊断价值。方法选择96例直肠癌患者,均进行3.0T动态对比增强磁共振成像检查。以手术病理结果作为诊断金标准,分析3.0T动态对比增强磁共振成像对直肠癌术前T分期的诊断结果及N分期的诊断结果、诊断效能。结果手术病理结果检出T1期共计12例、T2期共计20例、T3期共计46例、T4期共计18例。3.0T动态对比增强磁共振成像对直肠癌术前T分期的诊断准确率为95.83%(92/96)。手术病理结果(金标准)检出N0期共计20例、N1期共计45例、N2期共计31例。3.0T动态对比增强磁共振成像对直肠癌术前N分期的诊断准确率为88.54%(85/96)。对于N0分期,敏感度为80.00%(16/20)、特异度为96.05%(73/76)。对于N1分期,敏感度为88.89%(40/45)、特异度为88.24%(45/51)。对于N2分期,敏感度为93.55%(29/31)、特异度为96.92%(63/65)。结论直肠癌术前分期诊断中应用3.0T动态对比增强磁共振成像具有较高的诊断效能。
Objective To explore the diagnostic value of 3.0T dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in preoperative staging of rectal cancer.Methods A total of 96 patients with rectal cancer were selected and all underwent 3.0T dynamic contrast-enhanced magnetic resonance imaging examination.Using surgical pathology results as the diagnostic gold standard,this study analyzed the diagnostic accuracy and efficacy of 3.0T dynamic contrast-enhanced MRI for preoperative T staging and N staging of rectal cancer.Results The surgical pathology results revealed a total of 12 cases in stage T1,20 cases in stage T2,46 cases in stage T3,and 18 cases in stage T4.The diagnostic accuracy rate of 3.0T dynamic contrast-enhanced magnetic resonance imaging for preoperative T staging of rectal cancer was 95.83%(92/96).The surgical pathological results(gold standard)detected a total of 20 cases in stage N0,45 cases in stage N1,and 31 cases in stage N2.The diagnostic accuracy rate of 3.0T dynamic contrast-enhanced magnetic resonance imaging for preoperative N staging of rectal cancer was 88.54%(85/96).For N0 staging,the sensitivity was 80.00%(16/20)and the specificity was 96.05%(73/76);for N1 staging,the sensitivity was 88.89%(40/45)and the specificity was 88.24%(45/51);for N2 staging,the sensitivity was 93.55%(29/31)and the specificity was 96.92%(63/65).Conclusion The application of 3.0T dynamic contrast-enhanced magnetic resonance imaging in the preoperative staging of rectal cancer has a relatively high diagnostic efficacy.
