摘要
目的 探讨经颅多普勒超声联合脑脊液细胞学检查在结核性脑膜炎诊断及病情评估中的应用。方法 选取2024年5月至2025年4月本院收治的45例结核性脑膜炎患者为观察组,选取同期40例的非结核性脑膜炎患者为对照组。两组患者均接受脑脊液及经颅多普勒超声检查,比较两组超声参数及脑脊液细胞学指标,分析其诊断结核性脑膜炎的价值及其与疾病分期的关系。结果 观察组的WBC、 PRO、 PSV、 MFV、 EDV高于对照组,LY低于对照组(P<0.05)。各指标联合检测诊断结核性脑膜炎的AUC高于WBC、 LY、 PRO、 PSV、 MFV、 EDV单独诊断(P<0.05)。与Ⅱ~Ⅲ期患者相比,Ⅰ期患者的WBC、 PRO、 PSV、MFV、 EDV较低,LY较高(P<0.05)。Spearman相关性分析显示,WBC、 PRO、 PSV、 MFV、 EDV与结核性脑膜炎分期呈正相关,LY与结核性脑膜炎分期呈负相关(P<0.001)。结论 脑脊液细胞学指标与经颅多普勒超声参数联合应用可提高对结核性脑膜炎的诊断价值,且与患者的疾病分期关系密切。
Objective To explore the application of transcranial Doppler ultrasound combined with cerebrospinal fluid cytological examination in the diagnosis and condition assessment of tuberculous meningitis.Methods 45 patients with tuberculous meningitis admitted to our hospital from May 2024 to April 2025 were selected as observation group,and 40 patients with non-tuberculous meningitis during the same period were selected as control group.Both groups underwent cerebrospinal fluid and transcranial Doppler ultrasound examinations.The ultrasound parameters and cerebrospinal fluid cytological indicators of the two groups were compared to analyze the value of these parameters in diagnosing tuberculous meningitis and their relationship with the disease stage.Results The WBC,PRO,PSV,MFV,and EDV of the observation group were higher than those of the control group,while the LY was lower than that of the control group(P<0.05).The AUC of combined detection for diagnosing tuberculous meningitis was higher than that of the single detection of WBC,LY,PRO,PSV,MFV,and EDV(P<0.05).Compared with theⅡ~Ⅲstage patients,the WBC,PRO,PSV,MFV,and EDV of theⅠstage patients were lower,while the LY was higher(P<0.05).Spearman correlation analysis showed that WBC,PRO,PSV,MFV,and EDV were positively correlated with the stage of tuberculous meningitis,while LY was negatively correlated with the stage of nuclear meningitis(P<0.001).Conclusions The combined application of cerebrospinal fluid cytological indicators and transcranial Doppler ultrasound parameters can improve the diagnostic value for tuberculous meningitis,and is closely related to the disease stage of the patients.
