摘要
目的 探讨AngioJet机械血栓清除术联合导管接触溶栓在下肢深静脉血栓(DVT)形成中的应用效果。方法 回顾性分析2021年1月至2024年12月我院收治的95例下肢DVT患者的临床资料,按治疗方式不同分为对照组(48例)与观察组(47例)。对照组实施导管接触溶栓治疗,观察组实施AngioJet机械血栓清除术联合导管接触溶栓治疗。对比两组溶栓疗效、围术期指标、患肢周径、凝血功能、不良反应。结果 观察组溶栓总有效率高于对照组(P<0.05)。与对照组相比,观察组的溶栓时间、住院时间、肿胀消退时间均较短,尿激酶总用量较低,血栓清除率较高(P<0.05)。治疗后,观察组的膝上差、膝下差及D-二聚体、纤维蛋白原(FIB)水平均低于对照组(P<0.05)。两组均未发生严重不良反应。结论 AngioJet机械血栓清除术联合导管接触溶栓治疗可显著提升下肢DVT患者的溶栓疗效,减少溶栓药物用量,缩短住院时间,缩小患肢周径差,有助于改善凝血功能,且具有良好的有效性和安全性。
Objective To investigate the effect of AngioJet mechanical thrombectomy combined with catheter-directed thrombolysis(CDT)on patients with lower extremity deep vein thrombosis(DVT).Methods The clinical data of 95 DVT patients admitted to our hospital from January 2021 to December 2024 were retrospectively analyzed.Based on the treatment method,they were divided into two groups:48 patients who underwent CDT alone were assigned to the control group,and 47 patients who underwent AngioJet mechanical thrombectomy combined with CDT were assigned to the observation group.The thrombolytic efficacy,perioperative indicators,affected limb circumference,coagulation function,and adverse reactions were compared between the two groups.Results The total effective rate of thrombolysis in the observation group was higher than that in the control group(P<0.05).The thrombolysis time,hospital stay,and swelling resolution time in the observation group were shorter than those in the control group,the total dosage of urokinase used was less than that in the control group,and the thrombus clearance rate was higher than that in the control group(P<0.05).After treatment,the differences in circumference above and below the knee in the observation group were smaller than those in the control group,and the levels of D-dimer and fibrinogen(FIB)were lower than those in the control group(P<0.05).No severe adverse reactions occurred in either group.Conclusions AngioJet mechanical thrombectomy combined with CDT can significantly improve thrombolytic efficacy,reduce the dosage of thrombolytic drugs,shorten hospital stays,reduce the circumference difference of the affected limb,help improve coagulation function,and demonstrates good effectiveness and safety.
