摘要
目的探讨更年期异常子宫出血(AUB)患者应用葆宫止血颗粒联合甲羟孕酮、左炔诺孕酮宫内释放系统(LNG-IUS)治疗的临床效果。方法选取2022年01月-2025年01月在本院进行治疗的更年期AUB患者82例,随机数字表法将患者分为常规西医组和联合组,每组各41例。常规西医组患者采用LNG-IUS联合甲羟孕酮治疗,联合组在常规西医组基础上采用葆宫止血颗粒治疗,连续治疗2个月。在治疗后,比较患者疗效、子宫内膜厚度、血红蛋白、雌激素水平[雌二醇(E_(2))、黄体生成素(LH)]、炎症因子水平[C反应蛋白(CRP)、白介素-4(IL-4)]、不良反应发生率。结果治疗2个月后,联合组患者治疗有效率高于常规西医组(P<0.05)。与治疗前比较,治疗后常规西医组和联合组患者子宫内膜厚度降低,血红蛋白水平升高,治疗2个月后,联合组患者子宫内膜厚度低于常规西医组,血红蛋白水平高于常规西医组(P<0.05)。与治疗前比较,治疗后常规西医组和联合组患者E_(2)、LH水平均降低,治疗2个月后,联合组患者E_(2)、LH水平均低于常规西医组(P<0.05)。与治疗前比较,治疗后常规西医组和联合组患者CRP、IL-4水平降低,治疗2个月后,联合组患者CRP、IL-4水平低于常规西医组(P<0.05)。常规西医组和联合组患者不良反应发生率比较无差异(P>0.05)。结论葆宫止血颗粒联合甲羟孕酮、LNG-IUS治疗提高更年期异常子宫出血患者疗效,改善患者子宫内膜厚度、血红蛋白、雌激素水平。
Objective To explore the clinical curative effect of Baogong Zhixue granule combined with medroxyprogesterone and levonorgestrel intrauterine system(LNG-IUS)in patients with menopausal abnormal uterine bleeding(AUB).Methods A total of 82 patients with menopausal AUB treated in the hospital were enrolled between January 2022 and January 2025.According to random number table method,they were divided into routine western medicine group and combination group,41 cases in each group.The routine western medicine group was treated with LNG-IUS combined with medroxyprogesterone,while combination group was treated with Baogong Zhixue granules on basis of routine western medicine group for 2 months.After treatment,curative effect,endometrial thickness,hemoglobin,estrogens[estradiol(E_(2)),luteinizing hormone(LH)],inflammatory factors[C-reactive protein(CRP),interleukin-4(IL-4)]and the incidence of adverse reactions were compared.Results After 2 months of treatment,response rate of treatment in combination group was higher than that in routine western medicine group(P<0.05).After 2 months of treatment,endometrial thickness was decreased in both groups,which was lower in combination group,and hemoglobin level was increased,which was higher in combination group(P<0.05).After 2 months of treatment,levels of E_(2) and LH were decreased in both groups,which were lower in combination group(P<0.05).After 2 months of treatment,levels of CRP and IL-4 were decreased in both groups,which were lower in combination group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Baogong Zhixue granule combined with medroxyprogesterone and LNG-IUS can improve curative effect,endometrial thickness,levels of hemoglobin and estrogens in patients with menopausal AUB.
