摘要
通过分析肝与肺的相关性,据此阐释慢性阻塞性肺疾病合并焦虑抑郁的病因病机,探讨"肝肺同调"治疗慢性阻塞性肺疾病合并焦虑抑郁状态的可行性,以期为临床治疗提供更多参考。中医理论认为,肝与肺经络相通,生理病理基础与临床表现方面密切相关。慢性阻塞性肺疾病属于中医"肺胀"范畴,初起病位在肺。肝升肺降是维持人体气机平衡的关键,咳喘与肝密切相关。焦虑抑郁隶属于中医"郁证"范畴,肝气郁滞是其根本病因。故治疗慢性阻塞性肺疾病合并焦虑抑郁可肝肺同调,以调畅气机为根本,同时和气血,化痰瘀并结合辨证加减治疗,疗效显著。
By analyzing the correlation between the liver and the lungs,this review elucidates the etiology and pathogenesis of chronic obstructive pulmonary disease(COPD)complicated by anxiety and depression.It explores the feasibility of using the"liver-lung coordination"theory to treat COPD complicated by anxiety and depression,aiming to provide more evidence for clinical treatment.According to traditional Chinese medicine(TCM)theory,the liver and lungs are interconnected through their meridians,with a close relationship in both physiological and pathological aspects as well as clinical manifestations.COPD is classified as'lung distension'in TCM pathology,with primary manifestation in the lung system during early stages.The ascent of liver qi and descent of lung qi are pivotal in maintaining the dynamic balance of human qi,and as such,cough and asthma are closely related to liver function.Anxiety and depression are categorized under"stagnation syndrome"in TCM,with liver qi stagnation being the fundamental cause.Therefore,in treating COPD complicated by anxiety and depression,the liver and lungs can be regulated simultaneously.The treatment fundamentally focuses on regulating and smoothing qi dynamics,while also harmonizing blood and resolving phlegm stasis,combined with modifications based on pattern differentiation,achieving significant therapeutic efficacy.
