摘要
目的分析DIP支付方式改革后贵州省公立中医医院面临的困境,提出应对策略。方法采用目的性抽样的原则,2025年6月选取贵州省4家三级甲等中医医院的32名医护人员、管理人员开展半结构式访谈。基于扎根理论指导对访谈资料进行梳理、编码和整合。结果根据32份访谈资料归纳出32个初始范畴、10个从属范畴、4个主范畴。得出DIP付费对中医特色治疗配套政策支持不足、DIP付费方式与中医治疗理念融合困难、医疗服务供需矛盾突出、医院管理创新能力滞后4个维度的困境节点。结论中医医院要重视与医保的协同和沟通,推动实现中医药流程标准化,完善慢性病全程管理,加强信息化建设,建立良好的人才培养机制,以更好地适应DIP支付方式改革需求。
Objective To analyze the predicaments faced by public Traditional Chinese Medicine hospital in Guizhou Province after the implementation of the Diagnosis-Intervention Packet(DIP)payment reform,and to propose corresponding strategies.Methods Following the principle of purposive sampling,a total of 32 medical staff and administrators from 4 Grade A tertiary TCM hospitals in Guizhou Province were selected for semi-structured interviews in June 2025.Guided by grounded theory,the interview data were systematically sorted,coded,and integrated.Results Based on 32 interview data,32 initial categories,10 subordinate categories and 4 main categories were extracted.Four predicament nodes were identified,including insufficient policy support for Traditional Chinese Medicine characteristic treatments under DIP payment,difficulty in integrating DIP payment with Traditional Chinese Medicine treatment concepts,prominent contradiction between supply and demand of medical services,lagging management and innovation capabilities of hospitals.Conclusion Traditional Chinese Medicine hospitals should attach importance to the coordination and communication with medical insurance,promote the standardization of Traditional Chinese Medicine processes,improve the whole-process management of chronic diseases,strengthen information construction,and establish a good talent training mechanism,and management and better adapt to the requirements of DIP payment reform.
