低频电子脉冲治疗对脊髓损伤后神经源性膀胱尿潴留患者排尿频次及尿路感染的影响

摘要 目的 研究低频电子脉冲疗法对脊髓损伤(SCI)所致神经源性膀胱尿潴留患者的排尿功能及尿路感染发生率的影响。方法 采用随机对照试验设计,纳入2021年3月至2024年4月在郑州大学第一附属医院住院治疗的83例SCI后神经源性膀胱尿潴留患者作为研究对象,随机分为对照组41例和观察组42例。对照组接受基础治疗以及膀胱功能训练联合常规间歇导尿,观察组在对照组基础上加用低频电子脉冲治疗。评估两组患者的临床疗效、排尿功能参数、尿动力学指标变化及尿路感染发生情况。结果 治疗结束后,观察组的临床总有效率[95.24%(40/42)]优于对照组[78.05%(32/41)](P<0.05);与治疗前相比,两组患者治疗后日单次平均尿量、逼尿肌收缩压及尿流速率均提升(P<0.05),且观察组改善程度更显著(P<0.05);同时,两组每日排尿次数、漏尿次数、膀胱残余尿量及膀胱内压力均较治疗前下降(P<0.05),观察组下降幅度更大(P<0.05);观察组尿路感染发生率[4.76%(2/42)]低于对照组[21.96%(9/41)](P<0.05)。结论 低频电子脉冲疗法能显著改善SCI后神经源性膀胱尿潴留患者的临床症状,优化尿动力学参数,降低尿路感染风险,有助于膀胱功能恢复。 Objective To investigate the effects of low-frequency electronic pulse therapy on urinary frequency and urinary tract infection in patients with neurogenic bladder urinary retention after spinal cord injury(SCI).Methods Between March 2021 and April 2024,83 SCI patients with neurogenic bladder complications were enrolled from the First Affiliated Hospital of Zhengzhou University and randomly allocated to a control group(41 patients)and an observation group(42 patients).The control group received standard care and bladder rehabilitation exercises plus conventional intermittent catheterization,while the observation group received additional low-frequency electrical pulse therapy.Treatment outcomes,voiding patterns,urodynamic parameters,and urinary infection rates were analyzed and compared between both groups.Results After the treatment,the total clinical effective rate of the observation group[95.24%(40/42)]was better than that of the control group[78.05%(32/41)](P<0.05).Compared with before treatment,the average daily single urine output,detrusor systolic pressure and urine flow rate of patients in both groups increased after treatment(P<0.05),and the improvement in the observation group was more significant(P<0.05).Meanwhile,the daily frequency of urination,frequency of urinary leakage,residual urine volume in the bladder and intravesical pressure in both groups decreased compared with those before treatment(P<0.05),and the decrease in the observation group was greater(P<0.05).The incidence of urinary tract infection in the observation group[4.76%(2/42)]was lower than that in the control group[21.96%(9/41)](P<0.05).Conclusion Low-frequency electronic pulse therapy can significantly improve the clinical symptoms of patients with neurogenic bladder urinary retention after SCI,optimize urodynamic parameters,reduce the risk of urinary tract infection,and contribute to the recovery of bladder function.
