反复种植失败患者子宫内膜微生物群分析

Analysis of endometrial microbiota in patients with recurrent implantation failure

摘要 目的 探讨反复种植失败(recurrent implantation failure,RIF)患者子宫内膜微生物群的结构特征。方法 收集2024年9月—2025年6月于中国人民解放军总医院第七医学中心生殖医学中心就诊69例辅助生殖患者种植窗期子宫内膜组织样本。应用Illumina MiSeq平台进行16S核糖体RNA(16S ribosomal RNA,16S rRNA)基因高通量测序。利用α/β多样性分析、线性判别分析效应量分析及重建未观测状态对群落进行系统发育研究功能预测探讨菌群差异及功能特征。结果 RIF组的Chao1指数显著高于对照组(P<0.01);主坐标分析显示两组样本间菌群结构分离趋势明显(P<0.01)。属水平比较显示,RIF组乳酸杆菌属相对丰度显著降低[5.56(0.15~46.26)%vs. 64.26(14.19~83.12)%],而大肠杆菌[6.63(1.69~13.41)%vs. 2.23(0.77~5.68)%]及葡萄球菌属[0.31(0.07~0.82)%vs. 0.17(0.02~0.26)%]显著升高(均P<0.05);线性判别分析效应量分析提示RIF组富集肠球菌属、葡萄球菌属及大肠杆菌等菌群。差异显著的同源基因簇功能主要涉及物质转运、能量代谢及核酸修复;能量代谢、氨基酸代谢、膜转运、碳水化合物代谢及辅因子与维生素代谢等途径在RIF组显著上调(P<0.01)。结论 RIF患者子宫内膜菌群整体结构紊乱,优势乳酸杆菌属显著减少并伴随条件致病菌增多,其微生物功能潜能主要集中于能量与代谢相关通路。 Objective To investigate the structural characteristics of the endometrial microbiota in patients with recurrent implantation failure(RIF).Methods Endometrial tissue samples of 69 patients from the Seventh Medical Center of Chinese PLA General Hospital undergoing assisted reproductive treatment were collected during the implantation window.The high-throughput sequencing of the 16S ribosomal RNA(16S rRNA)was performed using the Illumina MiSeq platform.Differences in microbial composition and functional characteristics were analyzed usingα/β-diversity analysis,linear discriminant analysis effect size(LEfSe)analysis,and phylogenetic investigation of communities by reconstruction of unobserved states(PICRUSt).Results The Chao1 index of the RIF group was significantly higher than that of the control group(P<0.01).Principal coordinates analysis revealed distinct separation trend in microbial community structure between the two sample groups(P<0.01).At the genus level,the relative abundance of Lactobacillus was markedly reduced in the RIF group[5.56(0.15−46.26)%vs.64.26(14.19−83.12)%],while the relative abundance of Escherichia[6.63(1.69−13.41)%vs.2.23(0.77−5.68)%]and Staphylococcus[0.31(0.07−0.82)%vs.0.17(0.02−0.26)%]were significantly increased(all P<0.05).LEfSe analysis further identified enrichment of bacterial taxa,such as Enterococcus,Staphylococcus,and Escherichia in the RIF group.Functions of significantly different orthologous gene clusters primarily involved substance transport,energy metabolism,and nucleic acid repair.Significant upregulation of metabolic pathways,including energy metabolism,amino acid metabolism,membrane transport,carbohydrate metabolism,and cofactor and vitamin metabolism were found in the RIF group(P<0.01).Conclusion Patients with RIF exhibit a disrupted endometrial microbial structure characterized by depletion of the dominant beneficial genus Lactobacillus accompanied by an increase in conditional pathogens.The functional potential of the RIF microbiota is primarily enriched in energy and metabolism-related pathways.
