摘要
目的描述2023年山东省日照市首例猴痘确诊病例的发病过程、流行病学特征和疫情处置过程,分析传播原因,为猴痘科学防控提供参考依据。方法以2024年9月21日山东省日照市发现报告的首例猴痘确诊病例及其接触人员为调查对象。按照《猴痘防控方案》和《山东省猴痘防控工作方案》进行流行病学调查,通过询问病史,观察病例临床症状及体征,采集病例疱疹硬痂表面及分泌物、咽拭子、血液等标本进行实时荧光定量反转录聚合酶链式反应(RT-PCR)检测。结果病例于2023年9月9日和9月16日在日照市分别发生多次婚外性行为,9月20日发现生殖部位出现皮疹并就诊,从暴露到发病的间隔时间为4 d。当日检测其疱疹液猴痘病毒阳性,基因测序显示病例感染的猴痘病毒属西非系C.1.1分支。采集病例枕头和被褥等床上用品,卧室门把手和灯开关等高频接触部位共2份环境样本,核酸检测均为阳性。病例的接触者健康监测无异常。结论证实异性性行为传播猴痘,存在潜伏期低于5 d。病例的生活环境猴痘病毒污染,一般接触感染风险较低。
Objective To describe the illness course,epidemiological characteristics and response process of the first confirmed case of monkeypox in Rizhao city of Shandong province in 2023,analyze the causes of transmission,and provide evidence for the prevention and control of monkeypox.Methods The first confirmed case of monkeypox reported in Rizhao city,Shandong province on September 21,2024,and its contacts were selected as the study subjects.Based on the“Monkeypox Prevention and Control Program”and“Monkeypox Prevention and Control Work Program of Shandong Province”,epidemiological investigations were conducted,by inquiring the medical history,observing the clinical symptoms and signs of the patient,collecting the surface and secretions of the patient's herpes crusts,throat swabs,blood and other specimens for real⁃time fluorescence quantitative reverse transcription polymerase chain reaction(RT⁃PCR)detection.Results The case had multiple extramarital sexual encounters in Rizhao city on September 9 and 16,2023.On September 20,the case detected rash in the genital area and went to hospital.Interval from exposure to morbidity was 4 days.The case's herpes fluid was positive for the nucleic acid of mpox virus.The genetic sequencing results showed that the mpox virus isolated from the case belonged to the C.1.1 branch of the West African strain.Two environmental samples,including bedding such as pillows and bedclothes,and high⁃frequency contact areas such as bedroom doorknobs and light switches,were collected,and they were all positive.No abnormalities in health surveillance of contacts of the case.Conclusions Heterosexual transmission of monkeypox had been confirmed,with an incubation period of less than 5 days.The living environment of these cases was generally contaminated with monkeypox virus,and the risk of infection through general contact was low.
