摘要
目的研究新型冠状病毒感染(COVID-19)对甲状腺功能产生的影响。方法回顾性分析2022年11月至2023年1月东部战区总医院收治的COVID-19患者30例(COVID-19组),并收集同时期的非COVID-19的一般肺炎患者32例(一般肺炎组)及肺部正常者37例(肺部正常组),分析其甲状腺功能差异及与临床资料的相关性。结果COVID-19组甲状腺功能异常者(66.7%)较一般肺炎组(40.6%)及肺部正常组(29.7%)增加(P=0.009),COVID-19组患者游离三碘甲腺原氨酸(FT3)、促甲状腺激素(TSH)水平明显较低(Z=18.613,P<0.05;F=3.688,P<0.05)。在COVID-19组患者中,C反应蛋白(CRP)与三碘甲状腺原氨酸(T3)呈负相关(r_(s)=-0.301,P=0.012),淋巴细胞计数与TSH呈正相关(r_(s)=0.374,P=0.042),清蛋白与T3、FT3呈正相关(r_(s)=0.507,P=0.011;r_(s)=0.595,P=0.001),三酰甘油与游离甲状腺素(FT4)呈负相关(r_(s)=-0.582,P=0.001),凝血酶原时间、国际标准化比值(INR)与TSH呈正相关(r_(s)=0.407,P=0.026;r_(s)=0.440,P=0.015)。结论COVID-19患者更易出现甲状腺功能异常,表现为TSH、FT3水平下降。甲状腺损伤程度与CRP、血浆清蛋白、三酰甘油、淋巴细胞计数、凝血酶原时间、INR等指标相关。
Objective To investigate the impact of the novel coronavirus(COVID-19)on thyroid function.Methods Retrospective analysis was conducted on 30 patients with COVID-19(COVID-19 group)admitted to the General Hospital of Eastern Theater Command from November 2022 to January 2023,32 patients with non-COVID-19 pneumonia(general pneumonia group)and 37 healthy subjects(normal control group)during the same period.The differences in thyroid function among the groups and their correlations with clinical data were analyzed.Results The proportion of patients with abnormal thyroid function in COVID-19 group(66.7%)was higher than that in the general pneumonia group(40.6%)and the normal control group(29.7%)(P=0.009).The levels of free triiodothyronine(FT3)(Z=18.613,P<0.05)and thyroid stimulating hormone(TSH)(F=3.688,P<0.05)in the COVID-19 group were significantly lower.In the COVID-19 group,C-reactive protein(CRP)was negatively correlated with triiodothyronine(T3)(r_(s)=-0.301,P=0.012),lymphocyte count was positively correlated with TSH(r_(s)=0.374,P=0.042),albumin was positively correlated with T3(r_(s)=0.507,P=0.011)and FT3(r_(s)=0.595,P=0.001),triglyceride was negatively correlated with free tetraiodothyronine(FT4)(r_(s)=-0.582,P=0.001),and prothrombin time(r_(s)=0.407,P=0.026),international normalized ratio(INR)(r_(s)=0.440,P=0.015)were positively correlated with TSH.Conclusion Patients with COVID-19 are more prone to thyroid dysfunction,characterized by decreased levels of TSH and FT3.In the COVID-19 group,the degree of thyroid damage is related to indicator_(s) such as CRP,plasma albumin,triglycerides,lymphocyte count,prothrombin time and INR.
