Mini-CEX联合DOPS评价角色互换模式在口腔颌面外科住培教学中的应用效果

摘要 目的探讨角色互换模式在口腔颌面外科住院医师规范化培训(简称“住培”)教学中的应用效果及可行性。方法选取2023年11月至2024年11月在湖南中南大学湘雅口腔医院口腔颌面外科住培轮转的102名口腔医学专业型研究生为研究对象,采用随机数表法分为对照组和观察组。对照组(n=51)采用传统教学模式,观察组(n=51)采用角色互换教学模式。两组分别在入科及出科时采用迷你临床演练评估(Mini-CEX)和操作技能直接观察评估(DOPS)进行教学效果评价,并对教学满意度实施问卷调查。结果入科时两组Mini-CEX评分比较,差异无统计学意义(P>0.05),出科时两组Mini-CEX评分较入科时提高,观察组的总分及医疗面谈等6项评分高于对照组,差异有统计学意义(P<0.05)。入科时两组DOPS评分的比较,差异无统计学意义(P>0.05),出科时两组DOPS评分较入科时提高,观察组的总分及操作适应证等8项评分高于对照组,差异有统计学意义(P<0.05)。问卷调查显示,出科时观察组教学满意度评分的总分及授课形式等8项评分高于对照组,差异有统计学意义(P<0.05)。结论在口腔颌面外科住培教学中引入角色互换模式,可在一定程度上提升口腔医学专业型研究生的专业素养,获得良好的教学满意度,可考虑在口腔颌面外科住培教学工作中进一步推广应用。 Objective To explore the effect and feasibility of role exchange model in the teaching of oral and maxillofacial of residency standardized training(referred to as residency training for short).Methods A total of 102 graduate students majoring in stomatology who were undergoing rotation in the Department of Oral and maxillofacial Surgery,Xiangya Stomatology Hospital,Central South University,Hunan,from November 2023 to November 2024 were selected as the research objects,and were divided into control group and observation group by random number table method.The control group(n=51)adopted the traditional teaching mode,and the observation group(n=51)adopted the role exchange teaching mode.The two groups underwent miniclinical evaluation exercise(Mini-CEX)and direct observation of procedural skill(DOPS)assessment at admission and graduation to evaluate the teaching effect respectively,and a questionnaire survey on teaching satisfaction was conducted.Results At admission,there was no statistically significant difference in Mini-CEX scores between the two groups(P>0.05).At graduation,Mini-CEX scores in both groups were higher than those at admission.The total score and scores for six items(including medical interviews)in the observation group were significantly higher than those in the control group(P<0.05).No statistically significant difference was observed in DOPS scores between the two groups at admission(P>0.05).At graduation,DOPS scores in both groups had improved compared to admission.The total score and scores for eight items(including operational indications)in the observation group were significantly higher than those in the control group(P<0.05).The survey showed that the total score of teaching satisfaction and the scores of eight items such as teaching form in the observation group were higher than those in the control group at graduation,and the difference was statistically significant(P<0.05).Conclusion The introduction of role exchange model in oral and maxillofacial surgery residency training teaching can improve the professional quality of stomatology graduate students to a certain extent,and obtain good teaching satisfaction,it can be further popularized and applied in oral and maxillofacial surgery residency training teaching.
