摘要
目的分析利马前列素片联合内镜减压术对腰椎管狭窄症患者疼痛及腰椎功能的影响。方法回顾性分析2022年1月—2024年5月就诊于中国医科大学附属第四医院的80例腰椎管狭窄症患者临床资料,将单纯接受内镜减压术治疗的患者纳入对照组(38例),将接受利马前列素片联合内镜减压术治疗的患者纳入观察组(42例)。比较2组临床疗效,手术前后视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、日本骨科协会腰痛功能评价量表(JOA)评分、步行距离、血清炎性因子[白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)],不良反应。结果观察组总有效率为80.95%,高于对照组的60.53%(χ^(2)=4.063,P=0.044)。术后3个月,2组VAS评分、ODI评分较术前下降,JOA评分及步行距离较术前增加,且观察组改善幅度大于对照组(P<0.01);2组血清IL-6、TNF-α水平较术前下降,且观察组降低幅度大于对照组(P<0.01)。观察组与对照组不良反应总发生率比较差异无统计学意义(28.57%vs.21.05%,χ^(2)=0.602,P=0.438)。结论利马前列素片联合内镜减压术可缓解腰椎管狭窄症患者的疼痛症状,改善腰椎功能,减轻炎性反应,从而提高临床有效率,且未增加并发症及不良反应。
Objective To analyze the effect of limaprost tablets combined with endoscopic decompression on pain and lumbar function in patients with lumbar spinal stenosis.Methods The clinical data of 80 patients with lumbar spinal stenosis admitted to the Fourth Affiliated Hospital of China Medical University from January 2022 to May 2024 were retrospectively analyzed.The patients who received endoscopic decompression alone were included in the control group(38 cases),and the patients who received limaprost tablets combined with endoscopic decompression were included in the observation group(42 cases).The clinical efficacy,VAS scores,ODI scores,JOA scores,walking distance,serum inflammatory factors(IL-6,TNF-α)preoperative and postoperative,and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 80.95%,which was higher than 60.53%of the control group(χ^(2)=4.063,P=0.044).At 3 months after operation,VAS scores and ODI scores in the two groups decreased,JOA scores and walking distance increased,and the improvement in the observation group was greater than that in the control group(P<0.01).The serum levels of IL-6 and TNF-αin the two groups were lower than those before operation,and the decrease in the observation group was greater than that in the control group(P<0.01).There was no significant difference in the total incidence of adverse reactions between the observation group and the control group(28.57%vs.21.05%,χ^(2)=0.602,P=0.438).Conclusion Limaprost tablets combined with endoscopic decompression can relieve the pain symptoms of patients with lumbar spinal stenosis,improve lumbar function,reduce inflammatory response,so as to improve the clinical effective rate,and does not increase complications and adverse reactions.
