超声骨刀联合高速涡轮机拔除下颌阻生智齿的效果

Effect of Piezoelectric Surgery Combined with High-speed Handpiece during the Extraction of Impacted Mandibular Third Molars

摘要 目的:探讨超声骨刀联合高速涡轮机及单纯使用高速涡轮机拔除下颌阻生智齿对患者的影响。方法:选取2024年5月—2025年4月在江门市口腔医院行下颌阻生智齿拔除术的患者100例作为研究对象,按照随机数字表法分成试验组和对照组,各50例。对照组使用高速涡轮机进行手术,试验组则加用超声骨刀。比较两组手术时间、术中心理状态、疼痛、面积肿胀程度、张口受限情况以及并发症。结果:试验组手术时间较对照组长(P<0.05)。试验组术中心理状态优于对照组(P<0.05)。术后第3天,试验组疼痛评分、肿胀程度、开口受限程度优于对照组(P<0.05);两组术后第7天疼痛评分、肿胀程度、开口受限程度及并发症比较差异无统计学意义(P>0.05)。结论:超声骨刀联合高速涡轮机拔除下颌阻生智齿具有微创优势,可缓解术中心理状态,明显改善术后疼痛、肿胀、开口受限。 Objective:To investigate the effects on patients between using piezoelectric surgery combined with a high-speed handpiece and using a high-speed handpiece alone for the extraction of impacted mandibular third molars.Method:A total of 100 patients who underwent impacted mandibular third molars extraction at Jiangmen Municipal Stomatological Hospital from May 2024 to April 2025 were selected as the research subjects and divided into the experimental group and the control group according to the random number table method,with 50 cases in each group.The control group underwent surgery using a high-speed handpiece,while the experimental group was additionally treated with piezoelectric surgery.The operation time,intraoperative psychological state,degree of pain,facial swelling,limited mouth opening,facial complications of the two groups were compared.Result:The operation time of the experimental group was longer than that of the control group(P<0.05).The intraoperative psychological state of the experimental group was better than that of the control group(P<0.05).On the third day after the operation,the pain score,swelling degree and degree of opening restriction in the experimental group were better than those in the control group(P<0.05).On the 7th day after the operation,there were no statistically significant differences in the pain score,swelling degree,degree of opening restriction and complications between the two groups(P>0.05).Conclusion:The combined use of piezoelectric surgery and high-speed handpiece for mandibular impacted third molar extraction offers minimally invasive advantages,alleviates intraoperative psychological state,and significantly reduces postoperative pain,swelling,and opening restriction.
