早期肠内营养护理干预对老年重症肺炎患者预后的影响

摘要 目的:探讨早期肠内营养护理干预对老年重症肺炎(SP)患者预后的影响。方法:回顾性收集景德镇市第二人民医院ICU 2021年1月—2024年7月收治的92例老年SP患者的临床资料,经纳排标准剔除12例后,共纳入80例作为研究对象。根据护理方案将患者分为常规组(n=38,采用晚期肠内营养护理)和观察组(n=42,采用早期肠内营养护理),两组均干预至转出ICU。对比两组通气、氧合及营养指标、急性生理学和慢性健康状况评价Ⅱ(APACHEⅡ)、恢复情况、并发症、死亡率、护理满意度。结果:干预后,两组APACHEⅡ评分、PaCO_(2)均下降,且观察组低于常规组,氧合指数、白蛋白(ALB)均升高,且观察组高于常规组(P<0.05)。观察组的退热时间早于常规组,机械通气时间及ICU住院时间短于常规组,并发症发生率低于常规组(P<0.05)。观察组护理总满意度高于常规组(P<0.05)。治疗期间,两组均未出现死亡病例。结论:老年SP患者应用早期肠内营养护理干预可有效改善其临床指标和病情严重程度,缩短发热、机械通气及ICU住院时间,降低并发症发生率,提升护理满意度。 Objective:To explore the effect of early enteral nutrition nursing intervention on the prognosis of elderly patients with severe pneumonia(SP).Method:The clinical data of 92 elderly SP patients admitted to the ICU of the Second People's Hospital of Jingdezhen from January 2021 to July 2024 were retrospectively collected.After excluding 12 cases by the inclusion and exclusion criteria,a total of 80 cases were included as the study subjects.According to the nursing protocol,the patients were divided into the conventional group(n=38,received delayed enteral nutrition nursing)and the observation group(n=42,received early enteral nutrition nursing).Both groups were intervened until they were transferred out of the ICU.The ventilation,oxygenation and nutrition indicators,acute physiology and chronic health evaluationⅡ(APACHEⅡ),recovery status,complications,mortality rate and nursing satisfaction of the two groups were compared.Result:After the intervention,the APACHEⅡscores and PaCO_(2) in both groups decreased,and those in the observation group were lower than those in the conventional group,the oxygenation index and albumin(ALB)increased,and those in the observation group were higher than those in the conventional group(P<0.05).The fever reduction time of the observation group was earlier than that of the conventional group,the mechanical ventilation time and ICU hospitalization time were shorter than those of the conventional group,and the incidence of complications was lower than that of the conventional group(P<0.05).The total nursing satisfaction of the observation group was higher than that of the conventional group(P<0.05).During the treatment period,no deaths occurred in both group.Conclusion:Early enteral nutrition nursing intervention for elderly patients with SP can effectively improve their clinical indicators and the severity of the disease,shorten the time of fever,mechanical ventilation and ICU hospitalization,reduce the incidence of complications,and enhance nursing satisfaction.
